It’s unfortunate you chose to seek this avenue. But as it may be I will respond as follows:
First and foremost Yes, I absolutely had a License #[redacted] and Business Tax Certificate [redacted] while working at the office in [redacted]. I have not renewed them...

as I have not had a reason to. 
In October 2011 I had spoke to you in length on the phone prior to you coming in and consulting in person. During the in office consultation I walked you through the process, showed you before and after pictures and explained that your commitment to make your appointments were very important. I then asked if you had any questions or concerns and you said no but you will throughout and I assured you I would be there to answer them ANYTIME you needed. I gave you the cost of the series and agreed to make them in payments. I provided you with a detailed breakdown in form of a “Treatment Plan” (see below) for the procedures you were paying for. In all my years of work I have realized that some clients need a few more treatments at the end and I always am very willingly to give those freely. Below I have listed the procedures you came in for in date order as well as the Treatment Plan.

9 Visits and 4.5 Months Later
After your series we discussed scheduling additional appointments in order to target a few of the more stubborn areas. I recommended an additional Needling but not on the full face just a few of the deeper scars and the hypo-pigmentation. These appointments were at no extra charge to you. By this time I was finishing up my series with the other clients who started around the same time as you. I was not taking on other new clients because my practice in [redacted] was getting busier so back and forth travels and your additional appointments would not consist of every 2 weeks.

July 27, 2012                          emailed you to let you know I was working on trying to combine some touch ups on other clients and I would let you know subsequently scheduling for 8-26-12
August 26, 2012                     Traveled to [redacted] for your scheduled appointment- You cancelled last minute     
At this point I made you aware that I was leaving the ball in your court to reach out when you could make the time and I ALWAYS said I would be there. With empathy I knew you were stressing terribly with court issues and your ex-husband, remodeling the house, jobs, new grand baby etc… but I still made those trips to see you out of pocket and you cancelled last minute. In return I get your hurtful emails venting your frustrations of time going by and not seeing me BUT it was never because I did not diligently try.  I was clearly waiting for your communication. In many phone conversations I even offered for you to come to [redacted] any day of the week (all inclusive) if it made it easier to see you. I made every effort to accommodate you and all of your scheduling conflicts. I NEVER gave you the impression that I would not take care of you and your concerns and you even admitted that in several emails. I let you know as much as possible my availability when I knew I was coming in town even if it was to just see my sister so upon hearing from you in yet another irritated way (even though the ball was in your court) was upsetting. Again, I made every effort to accommodate you as expressed you had some open windows.

January 8, 2013                       You agree to January 8, 2013 but on January 3rd you had to CNX
February 13, 2013                   You emailed and said you could not commit to Feb 23rd because you did not did want it to interfere with any assignments because of your face looking funny. I told you we would not Needle and to PLEASE come anyways I will do another peel procedure just as maintenance and there is no downtime. After a phone conversation discussing this you agreed.

After seeing you on February 23, 2013 I once again left the ball in your court and reiterated that you can come to [redacted] anytime for treatment all inclusive. At our last appointment together you were upbeat and grateful for making the time to help your skin (because I will and always will). I have never let a client down I have worked with you with good intentions but I’M EXTREMELY SADDENDED BY YOUR FINAL ATTACK AFTER I HAVE BEEN SO DUE DILLIGENT TO TAKE CARE OF YOU. IN MY CONSULTATION I STRRESSED HOW PASSIONATE I AM FOR MY WORK AND NO MATTER WHAT YOU WILL HAVE RESULTS (AS SEEN IN PHOTO).
[redacted], my character does not match anything you are saying. In my 7 years plus in business I have never had one complaint. As you can see by the dates I did honor all your appointments In addition you received the 2 extra Peels on April 23, 2012 and February 23, 2013 as mentioned in your Treatment Plan and no charge.  The last thing I attempted time and again was to schedule another Needling to a few specific areas which are evident in the photo.
I’m truly sorry but I don’t refund for services rendered, however I will offer you a full credit of $3595 for services or to use towards a whole new series. There was definitely change to your skin for the better. Stress and other factors can take a toll on us inside and out. I know you have had you fare share. I would love to get started with you as soon as possible.
