from [redacted] and [redacted] (not Dr. [redacted] does not address any of the concerns expressed in my complaint.  I would also like the record to show that the only consent form I had reservations about signing was one stating that I consented to whatever treatment was offered and that I had all my questions answered by Dr. [redacted].  Since I had not yet seen the doctor, I explained to the staff person at the desk, that I didn't feel comfortable signing this until I had actually spoken to the doctor.  However, at this time I was not told that this would result in my not being seen by Dr. [redacted].  I only found that out once I was seated in the chair and being prepared for X-ray by his assistant.  The response does not address the discourteous and disrespectful treatment I received from Dr. [redacted].  It does not address the fact that Dr. [redacted] claims to be a member of the ADA and the ODA , but is not listed on their websites.  It does not address the fact that I was initially told my personal information could not be deleted but only moved to an inactive file.  It does not address the rudeness of being hung up on twice when I called the office.  And It does not address my request for a letter assuring me that my personal information entered on their on-line new patient intake form, which included my Social Security number, was deleted.  Because of the way I was treated, I was distrustful of their word and wanted that assurance in writing.  I still do not believe that in the age of identity theft my request for a letter of assurance is exaggerated or unreasonable.

Our office prices itself on patient privacy. We are proud of PCI compliance with monthly scanning to assure that our firewall, network, and credit card processing has all necessary security upgrades. Furthermore, our office also has AVG Business Security software on all workstations and the server. Our service is encrypted with SecureDoc software so that it would be useless to a thief. All back-ups are also encrypted and stored off site. We do not store any patient credit card numbers. No wireless access points exist into our network. While in our office, each employee has their own user login and password so that any malfeasance may be traced. No paper charting exists and all correspondence received form insurance companies is shred. Our office has seven employees. We have been a fixture in the community for 10+ years. We would recommend anyone concerned about identity theft be wary of signing HIPAA releases for national corporations which could possibly give anyone access to any number of private medical records. As far as our office is concerned, this person is not a patient of record and has no existing information in our system (i.e. it was deleted) that would be accessible by normal ordinary means.Sincerely,[redacted]

Due to Mr. [redacted] refusing to sign all consent forms, he was asked to leave our office. Since there was no treatment performed, he was removed/deleted from our system as ever being a patient of record.

I reviewed the response made by the business in reference to complaint ID [redacted], and find the resolution far less than fully acceptable.  Still, I will not pursue the matter any further.  While I appreciate Dr. [redacted]'s statement on patient privacy and the fact that my information has been deleted and would not be accessible by "normal, ordinary means," it is always the extraordinary skills of hackers that trouble me.  However, it appears clear to me that I will not get the letter I requested.  It also appears clear to me that he does not wish to respond to the other concerns I raised in regard to the discourteous and disrespectful way I was treated by him and the individual with whom I tried to communicate on the telephone.  Perhaps I should be rather grateful for this, however, because the way I was treated in his office has led me to find another one of the dentists in the community with whom I am very pleased.
