This letter is in response to your May 27, 2016, correspondence regarding the above referenced.The death claim proceeds of the life insurance policy referenced in the "Customer Experience Information" form were payable to three (3) beneficiariesAll three checks were mailed on the same day to the

November 30, 2015 In April of 2015 I took out a policy on my fiancé  [redacted] Weston, with Mutual Life Insurance. Mr. Anthony W[redacted] processed all my paperwork. I made it clear to Mr. W[redacted] that [redacted] and I were not married legally. [redacted] and I had what most would consider common law marriage, being that we were together for over 7 years and living together. However Mr. W[redacted] took it upon himself in selecting that [redacted] was my spouse, which to my understanding was the reason of me not being able to receive the insurance refund. I have suffered a great loss in losing my fiancé and now to deal with this. I made sure that at the time of me proceeding with the policy I asked all necessary questions, believing that Mr. W[redacted] had my best interest and knew what he was doing. He assured me that we did not have to be legally married.  Nevertheless Mutual Life Insurance was still deducting money from my paycheck every pay period. I was never informed that they needed more information from me. Now I am being penalized for a mistake that was on their end. The only time I was contacted was when I received a letter stating that I was not entitled to the refund. Since receiving the rejection letter, I have been trying to get answers and taking action on why I was rejected. The only answer that I have been getting is that it was due to the relationship status. It was supposed to be entered into the system as “common law” and not “spouse”.   This is one of two policies that I have with Mutual Life Insurance, which Mr. W[redacted] has processed and made mistakes on. My second policy that I took out was for my God-daughter. When processing this Mr. W[redacted] entered in the system that she was my Grand-daughter. Due to the mistakes that Mr. W[redacted] has made I had to also cancel that policy.             I took out life insurance for my loved ones so that god-forbid if anything happens, I or my family wouldn’t have to scramble around to pay for funeral services/expenses.  Life Insurance is very important and it is something that everyone needs. So for me to be going through this, due to Mr. Williams’s mistakes and lack of communication is totally unfair! I have suffered enough with his loss and I want this situation to be resolved right way. I feel that I am entitled to be getting what I am owed, which is what I opened the policy for.
