This company and the former employe r in NH that I got a broken nose from are the absolute worst ever to deal with! I never received an accident form at Dollar Tree on Loudon Road in Concord, NH I spoke with the person once at Sedgwick and via e-mail and never heard from him again And, then all of a sudden I am getting a letter pretty much accusing me of lying about breaking my nose and saying that it was from a previous health issue That was total [redacted] Dollar Tree should be reported to OSHA and the evening in question several female employees and myself were unloading freight from an wheeler that had made a delivery I was at the end of the line where you are supposed to put the boxes in their proper areas Suddenly, my foot caught according to my co-worker, Susanna when she found me My right foot was caught on a large U-Boat filled with boxes that was in the way The area that the stock is put in is not at all safe and should be reported One evening as I was trying to find alot of stock for a customer boxes fell on my head I am disgusted and livid that the people I worked with denied being witness to what happened I screamed as I bounced off the floor and broke my nose leaving blood every where They were at the front of the line and when they realized what happened came running They left me laying there while they first called the Manager, and then 9-1-1! I was taken by ambulance and treated for a messed up wrist, messed up ankle on my right foot and was in severe pain with my nose There was blood every where and a co-worker finally got toilet paper to slow the bleeding So their claim that I had a previous medical issue was [redacted] !

I had major knee surgery on right knee on 5/ I was approved for weeks of disability and spent that time working on strengthening my leg and obtaining a functioning Range of Motion My doctor submitted information times to Sedwick stating that I needed an additional week to insure that I had enough strength to "begin walking outside of the home" and enough range of motion to "begin to drive a car again." Sedwick clains to have never recieved the paperwork and my clain was denied The doctor faxed the informatino times Once I was standing right there and received validation that the informatino was recieved on Sedgwicks end I also faxed the information and received confirmation of receipt Ultimately Sedgwick denied the claim because of "failure to submit proof of continuing disability
