Hi,I have asked our Billing department to make a complete investigation of this complaint and let me know the findings.Please see the attached result of our investigation.After a thorough investigation,we determined the following:1.      We have the right patient and...

the correct insurance information that we obtained from NYU.2.      Shortly after the charges were paid by the patient’s insurance, we sent a patient invoice for the copay in the amount of $118.203.      The copay invoice was sent to a Brooklyn address, but was returned as undeliverable.  4.      The staff member who was in charge of resolving undeliverable mail performed a “skip trace” by looking up the patient against Accurint records.   Accurint is a paid skip tracing service with strict access and confidentiality guidelines; it is not open to thepublic.  The skip trace was NOT performed through [redacted].5.      The Accurint search yielded a hit for a person with the same name who lived in Chicago.  We know now that this is the person who filed the complaint with Revdex.com.   We also determined that the person with the same name in Chicago had a different DOBthan our patient.    Conclusion: Although the services were rendered to the right patient, and the correct insurance was billed, the patient copay invoice was sent to the wrong person.    The staff member who made the error is no longer with us.  However, the [redacted] in charge of that area was shown the issue and the effects of incorrect address assignment.  Going forward, the next person who will be put in charge of resolvingundeliverable mail will be trained appropriately in the use of Accurint searches.  We recommend that the person who made a complaint verify with their [redacted] carrier, and confirm that no claim was made against their policy.

Review: I received a bill from Senior Care Emergency Medical Services in the second week of May 2015 for ambulance services from KINGS COUNTY HOSPITAL (672) to NYU HOSPITALS CENTER (TISCH). I live in Chicago, IL and have no relatives or relations that live in New York. The charge was for $1,156 and it said that two payments of $400 and $72.80 were made through [redacted]. My first thought was this is medical fraud, given its prevalence presently. I've had to make fraud alerts on multiple accounts and call my actually health providers to report fraud. Days spent to try and get ahead of this. I get a hold of someone in customer service at Senior Care EMS and they tell me call billing and abruptly hang up. I get a hold of someone in billing at Senior Care EMS and I am told that a woman that worked there pulled my address off of [redacted]. That woman has since been fired and I should ignore the bill.

Let me restate that. She pulled my address off of [redacted]. She sent me a bill for an ambulance ride in a completely different state by pulling my address off of [redacted].

I searched for hours (quite comprehensively) on [redacted] and could not find my address. So how is it possible some employee for a small EMS company in the Bronx finds my address on [redacted] and then bills me for an ambulance ride when I live in Chicago?

I am assuming that this woman had access to the BCBS network (which I am a member of) and pulled my address from the network. This would seem like some violation of HIPAA. (Full disclosure: I am not a lawyer).

If she did somehow find my address on [redacted] (would like to know where she found it) or pull my information from the BCBS network - that is a despicable and gross violation of privacy. Its also a waste of my time and everyone else's time that I had to talk to make sure there was no fraud (any health accounts opened in my name).

If needed, I have the original bill and copies of the original.Desired Settlement: I would like to know exactly how this billed to me. Not "some woman looked your address up on [redacted]". I want a full investigation and explanation into how it happened. My address is not readily available on [redacted] unless Senior Care EMS has some sort of super-sleuthing [redacted] search tool. I would also like assistance with confirming that my information wasn't used in any way to open up fraudulent accounts.

Regardless of how the individual in question got my personal information, the individual's actions were a gross violation of privacy. The company's whole collections process is poorly supervised and as a result, I'll be reporting the business to all appropriate agencies who oversee such matters. Sincerely,
