On 7/2/14 the repair and final AOSE inspection was completed
On 7/14/14 [redacted] emailed our office with a list of service issues that he would like to be addressed sometime in July or in September since he would be away from home during the month of August-Attachment C
On 7/23/14 and 7/28/14, SES crews returned to the site to address the service issues listed in [redacted]'s 7/14/14 emailPer the signed contract terms, SES processed a final credit card payment on 7/31/14 (value $3,067.50) — Attachment D
On 7/31/14 [redacted] emailed our office, notifying us that he was disputing the final credit card charge, as well as bringing to our attention that a tank pump out was charged to his card but was never actually completed - Attachment E
After receipt of [redacted]'s email on 7/31/14, SES project manager Mike C** spoke to [redacted] and confirmed that he would be returning home in September – Attachment F
After completing our research on 9/3/14, SES verified that a pump out was not completed for this project and a credit was immediately issued to [redacted]'s card (value $325.00) – Attachment G
On 9/29/14 a copy of the AOSE completion package was mailed to [redacted]'s address
On 10/1/14 a copy of the AOSE completion package was delivered to the county Health department

Review: This vendor has shown a repeating history of not following through on items contractually promised. First, the vendor attempted to collect full payment for services not completed. Second, the vendor charged for services not performed. After refusal of payment, the vendor corrected the overcharge and performed necessary repairs. Lastly, vendor still has not delivered the final paperwork contractually required. Paperwork is now several months overdue. Vendor has been contacted a multitude of times over the period of interest and promised delivery with no follow through.Desired Settlement: Vendor should deliver promised paperwork.
