**.[redacted]Complaint Date 1/11/2014Corrected Date of Service 1/07/14**. [redacted] was a patient of Dr. [redacted] Shah, Neurology and Pain Management.**. [redacted] was sent a letter on January 17, 2014 stating upon review of his medical record, appointment compliance history, conversation with staff and other Shah Associates providers, the selection of another physician would be appropriate. He was told Dr. [redacted] Shah would be available for an emergency situation for the next 30 days. I have included a copy of the letter.Due to HIPAA standards, I am somewhat restricted on specific patient health information I can provide, but it was determined that **. [redacted] was non-compliant with his prescription regiment.Due to **. [redacted]’s insurance coverage (Medicare and Medicaid) there are no copays required nor is there any outstanding balance for 2014.I hope I have provided the information you needed.Please do not hesitate to contact me with any further informational needs.Very Respectfully,
