November 20, Dear [redacted] ,I am in receipt of your letter dated November 16,I will abide to your request of not writing names in keeping with the HIPAA guidelinesI am a participating provider with this patient"s insurance company I abide to their fees and subsequent adjustments Prior to starting the dental treatment on this patient, a pre-authorization was submitted as well as phone calls made to the Insurance company through the provider relations phone numberMy office currently is working with a supervisor in the provider relations department of the insurance companyProcedures codes are under review and we are waiting for their reply My office cannot do

I am rejecting this response because:  All I want is the credit that is due to me.  It is unfortunate that I had to get my dental insurance involved after I took the explanation of benefits to the dentist's billing person due to her stating that she "shredded" the EOB's she received.  So I have been waiting for over a month for a credit,  still in pain from the work I had done there AND the dentist is STILL BILLING for additional services from earlier this year(additional anesthesia) that I NEVER was informed of the extra charge for this that is not covered by insurance AND an exam for pain that I had because the crowns she placed in my mouth did not fit correctly.  I have had crowns in the past and my former dentist NEVER billed if I was uncomfortable with the fit and they had to be adjusted.
