April 27, 2015To Whom It May Concern:Our office has received several letters from this patient disputing both her balance and stating fraudulent charges have been billed in her name. Our billing department has confirmed with her insurance on February 3, 2015, with a reference number of [redacted],...

the amount owed to us is correct. Our billing representative also has spoken with supervisors from her insurance company to get a more clear statement of her benefits sent to the patient for a clearer understanding of her responsibility of payment.We have tried to contact the patient on several different occasions to discuss her correspondence mailed to our office. We have called and left messages asking for a return call on 2/3/15, 3/2/15, and 3/10/15. There has been no reply to those messages.The patient is also disputing charges from a date of service on January 2, 2015. She had come in on 12/26/14 but services were not rendered by a physician but by a nurse and were documented. She then came in on 1/2/15 in which services were rendered. We have documented both dates correctly to reflect the events each time.In an effort to have this on-going issue concluded, we are sending the patient a letter with confirmation of our ended physician-patient relationship and are clearing the charges off her account.Thank you for your help with this issue,Kyle WOffice Manager Shenandoah Urologic Specialists
