and bold print. Patient is a Medicare patient with a Medicare replacement plan. Return policy is also listed on this document.Second document is out patient privacy form signed by [redacted], patient. This document has billing information and indicates the contract for payment of equipment/service is made with you (patient), not the insurance company. This information is underlined.The second document also states the business will bill patient insurance/Medicare/Medicaid, but processing of patient insurance does not guarantee payment by the insurance company.

I am rejecting this response because:
We feel we weren't informed of the price, that was not filled in at the time I signed that form. [redacted] would never have agreed to pay that much as he has several ace bandages he could have used. But the fact that they took insurance info we thought it would be covered.He will pay for the one set he did use, but will return the other set with socks to be credited to clear the account. Please advise if acceptable and I will mail them back with a check.Thanks,[redacted] for [redacted]
