The information the consumer received from the Medical Records department was incorrect. That department has been educated on the correct process and how to handle possible issue's like this in the future. The consumer was listed as guarantor after the ex spouse informed us he was responsible and supplied all contact information. We did not receive his information from any physician office as previously stated. We did remove him as guarantor after the consumer contacted us and we researched this issue

I am rejecting this response because:They still did not produce the document or resolve the reason I why was placed as the gurantor on this accountThough now I have been removed as the gurantorThey still practice intimidation when I told the business to remove me as the gurantor they said I would have to pay the $29k bill that my insurance had already paidThis is the Chico Surgery Centers common practice is to intimidate people that do not know any that it is wrong to put someone as a gurantor without their knowledge

Review: The billing statement has me designated as the Guarantor. I have not signed or given my consent in any form in any way. When I brought this to the medical records department attention I was told if I did not submit I would be billed for the $29000 procedure. The portion that my insurance paid. Then the medical records department stated the Guarantor was referred to them by the referring doctor. They refused to give the name of the doctor doing the referring of the forms. The billing statement has my name as the guarantor but not my address. I have asked for this documentation but they have refused to let have them.Desired Settlement: A copy of the fraudulent form that shows the forgery.
