Dear Sir/Madam, The below matter was forwarded to my attention for response to youThis particular individual has never submitted a claim for warranty coverage to us I am told this person recently called in to our offices in Phoenix demanding warranty coverage and hung up after we asked for documentation showing proof of purchase in the form of a sales invoice showing that our products were purchased, the date, address of installation, and name of original purchaser and paid receipt and some photos showing what their concerns are To this date, we have not received any supporting information to allow us to determine whether this person bought any of our productsDuring the last several years after the economic turndown of and on, a significant number of our competitors went out of business, and persons call in or contact us for warranty coverage, only to find out the products were not ours and not sold by us or our dealers It is not uncommon for someone to call in and demand warranty coverage and when asked for proof that it is our products, they hang up and we never hear from them again because they did not buy our products but were hoping to get someone to fix whatever issues they hadIf this particular person wants to forward documentation to me via email, we will examine their situation in accordance with our warranty provisionsUntil that happens, we have no way of knowing if this is our product or whether the issues may or may not be covered by our warranty Also, I would like to note to you that we are not members of the Revdex.comWe have not agreed to your terms and conditions of how you operate, including your process of reporting complaints on businesses such as ours We would expect you not to publish any negative or derogatory information on our company unless you know for a fact it is true and accurate In this case, if you elect to publish any negative information about this particular matter and it turns out that this is not our product, then we will look to hold your company liable under the laws of defamation and slanderThis person is welcome to email or fax me the above information if they want us to consider their claim for warranty coverage If it turns out this person's products are ours and are covered under warranty, then we will honor our warrantyUntil we receive this basic and reasonable information, there is nothing we can do with respect to thisPlease let me know if you have any questionsRegards, Brian S [redacted] President

Te Dear Sir/Madam, The below matter was forwarded to my attention for response to you First of all, please note that we did not receive any sales receipt or paid invoice with the documents that were submitted, that show or establish that this person actually purchased any Slide-Lok brand products or that any Slide-Lok brand products were used on this person's floor The form this person submitted showing Slide-Lok markings is an estimate form This form is commonly used in the quoting process and given to interested individuals when they are in the process of considering purchasing Slide-Lok brand products The estimate form is not proof of purchase Many customers receive an estimate from one of our dealers and do not purchase our product while receiving quotes from other companies If this person can provide a sales receipt showing payment and purchase of Slide-Lok products, a credit card receipt or canceled check (both sides) showing payment for Slide-Lok brand products than we will review and reconsider their claim for warranty coverage Also, the estimate form is cut-off and does not show the entirety of the information that would have been on that form We would like to see the entire form, with the edges not omitted If this person can provide satisfactory proof of purchase, we will evaluate their warranty claim Please let me know if you have any questions Regards, Brian S [redacted] President cell [redacted] fax: [redacted] ll us why here

Very disappointing that a company is finding every excuse to not stand behind their product Attached is an email chain dating back to December that shows that the Company representative, Matt T*** acknowledged receipt of the information, requested measurements of the area of the garage floor in question, which were provided, along with pictures that clearly show the vendors product applied to our garage floorIn addition to the emails there were multiple discussions over the phone with MrT*** who always committed to providing a response I was never asked for a receipt (which was never provided by the vendor) or never advised that they could not see the entire quote form (which has been attached in two documents since their form is not standard x 11) Ten months later they are just now asking to see the edges of a form How ridiculous Do what is right, stand behind your Residential Garage Floor - Limited Lifetime Warranty If I never had the product installed why would I be asking for warranty coverage on a small section of the product that is failing? I look forward to a response that addresses the issue

Unless I am missing something in the documents that were attached to the files on your website link, there still has not been any receipt submitted showing payment for Slide-Lok brand products that were claimed as having been purchased. 
 
Our warranty is attached.  The warranty language has never changed.  The 2nd paragraph requires original sales order and receipt to make a warranty claim.
 
As stated before, the form submitted by the complaining person is an estimate form.  This form is used to provide potential customers with estimates for what our products might cost.  It is not proof of purchase.    Many people receive an estimate from one of our dealers and never actually purchase our products.
 
Please have this person submit a paid receipt - a canceled check (both sides), a credit card receipt, or credit card statement showing payment in a manner that conforms to and is consistent with what the person is claiming.  If proof of payment (purchase) is submitted, we will examine the claim in accordance with the terms of our warranty. 
 
Until we receive proof of purchase (paid receipt), we have no choice but to decline request for warranty coverage as this person has not substantiated that they did in fact purchase our products.
