This letter is in response to your request for information on our client, *** ***Ms*** booked an appointment for a free consultation at one of our many locations on 01/25/On that date, Ms*** came into our Hurst, Texas location and one of our management team sat down with Ms*** and explained our program in complete detail to her including all charges she would incur throughout her entire programAfter hearing this information, Ms*** decided to enroll in the program and, at this time, signed a contract (copy enclosed)Ms*** was a client referral, we did not solicit herHer friend *** *** told her about Slim4LifeMs*** is a client in LakeworthMs*** referred Ms*** to our programWe then proceeded with serviceWe took a complete medical history on Ms*** which she signed (copy enclosed), when the counselor asked MS*** if she had any allergies, she said noWe explained the preparation phase of our program to Ms***We took Ms***'s blood pressure, which was 130/84, and we took Ms***'s measurements, which were chest: 53.5, waist 53, hip: 57, thigh: and right arm: inchesMs*** picked out her starter supply of products, she signed for and left with $in productsI have included a copy of Ms***'s Disclosure Statement which is signedThis statement is where our management team discloses for each potential client the cost of their program, what is included in the cost, the cost of all mandatory products, and a list of all products included in their program price The Disclosure Statement plainly states, "Pricing for my weight loss program has been disclosed to me and I understand that Nutritional Su lements and roducts are a re uired art of m nutritional proqram and are not included in the proqram price except for your starter packAll sales are final." We have every person who has a consultation sign this statement so we do not have any confusion with what is included in the program price and the price of their products that are mandatory if they want Slim4Life to guarantee their weight loss resultsSlim4Life has five different programsWe do a complete study of our client's medical historyWe evaluate age, sex, medical conditions, medications taken, degree of obesity, and exercise level to determine which program is best suited for our client's needsWe also evaluate to determine if the client needs any modifications to the program we have determined would be best suited for each individual clientWe set up an individualized program for Ms*** on 01/26/cuse On 01/30/and 01/31/2017, Ms*** returned to the center and signed in and she was counseledOn 01/31/2017, Ms*** was counseled and attended her nutritional classAt nutritional class we explained Ms***'s individualized program to her that we had set up specifically for her and gave her detailed instructions on how to follow her individualized programOn 01/31/Ms*** purchased $in additional productsMs*** was given a 30% discount on all her products for buying her products in volume purchase for her entire programMs*** came in for a counseling session on 02/02/2017, 02/03/2017, 02/04/2017, 02/07/2017, 02/11/2017, 02/15/2017, 02/16/2017, 02/20/2017, 02/23/2017, 02/24/2017, 02/25/2017, 02/27/2017, 03/01/2017, 03/03/2017, 03/09/and 03/15/On 03/15/Ms*** told her counselor she was unhappy because she was allergic to soy, the counselor showed Ms*** her consultation sheet she filled out at the time of her consultation and where it asked about allergies she wrote codeine and did not write soy (copy enclosed)Our counselor also showed Ms*** when we took her medical history that she signed saying all the information was correct and also did not mention soy when asked about allergiesOn 03/15/Ms*** had lost a total of poundsMs*** had received counseling sessions In closing, Ms, *** signed a contract which plainly states our refund policy(TERMS AND CONDITIONS, RELEASE OF RESPONSIBILITY AND PERMISSION TO PROVIDE SERVICE) NON-REFUNDABLEALL NEW PROGRAMS INCLUDE ENROLLMENT AND SFEE OF $WHICH IS NON-REFUNDABLEI also understand that ALL SALES ARE FINAL and that any and all Slim Snacks and nutritional products are non-refundable and non-exchangeable once the Slim Snacks and nutritional products leave the centerI also understand once given my assigned nutritional program at the program explanation class, the program becomes NONREFUNDABLE IN ITS ENTIRETYI further understand that Slim4Life Weight Loss Centers are agreeing to provide me individual counseling up to visits per week during my program for the contracted weeks set forth in this agreement during the designated business hoursI hereby grant permission to employees at Slim4Life Weight Loss Centers to provide me with weight loss servicesI warrant to Slim4Life that I am in good health and fully able to participate in this program and do not require my Physician's release to do soI agree to hold the company, its offices, nurses, and employees completely harmless from any and all claims, liabilities, or damages for personal injuries which I may suffer directly, or indirectly, from and participation in the programEmployees of the company are not health care providers and cannot be expected to diagnose or treat individual health problemsI will address my health concerns to my personal physicianI understand that all weight loss treatment and program weeks run consecutively from my start date and that weight loss varies with individuals and how well they adhere to the program and therefore, this program in non-cancelable and non-refundableI hereby acknowledge that I understand the company's no refund and non-cancellation policy and that I have read, understood accept and agree to be bound by the terms and conditions of this agreement and I have received a copyI understand that I am responsible for the cost of ALL required Case Slim Snacks at a cost of $to $per box dependent on the volume purchased and that Slim Snacks and nutritional products other than those received in my started pack at the time of enrollment are not included in the program price*All Slim Snacks and nutritional products are non-refundable and non-exchangeable once the product leaves the centerThe client is responsible for the cost of all Slim Snacks and nutritional products, other than those given in my starter pack at the time of enrollmentI HAVE READ, UNDERSTOOD, ACCEPT AND AGREE TO THE ABOVE TERMS, CONDITIONS AND NON-CANCELATION AND NONREFUNDABLE POLICYMs***'s contract also states the exact cost associated with all products we sellOn the bottom of all contracts, you will see every product listed along with the cost of all productsThe contract also plainly state that all products are non-refundable and non-exchangeable once the product leaves the centerAll products we sell are consumedI am sure Ms*** would not want to purchase products from a company and consume them after they have been in someone else's car trunk, pantry, basement, or under their sinkProducts could be tampered with, not that Ms*** would tamper with them, but there are other people who would, So, for consumer safety, we will not allow returns because we will not resell any product that has left our centersAnd, as stated earlier, we have every client sign agreeing to this policy before they leave with any productsMs*** disputed her credit card chargesWe sent all of Ms***'s paperwork to the credit card company showing Ms*** did not tell Slim4Life she was allergic to soyShe did not write it on her consultation sheet under allergies nor on her medical history that she signed Her credit card company returned the money to Slim4LifeWhen Ms*** was notified of this she called our Hurst locationOur Area Supervisor, our Area Service Coordinator and our Area Director all spoke with Ms***We offered to refund all money she had paid for servicesWe offered to let her return all unopened products that contain soy with a doctor's note stating she was allergic to soyWe told her we would return all her products to the manufacturer and they would give us a partial refundWe would refund Ms, *** minus percent when she brought in a note from her doctor stating she was allergic to soyMs*** stated she had sold part of her products but would return the rest and she would continue her program without the soy productsWe thought Ms*** was satisfied and was going to be continuing with her programA few days later our Supervisor called the Hurst center to check up on Ms*** to see if she returned her unopened products and she had notWe called Ms*** again and she stated she decided to give all her products to the senior living home close to herThe Manager of Hurst location said at one time Ms*** had said she was going to sell the products to someone she knowsWe know we tried to work with Ms*** and Ms*** lost over pounds in weeks on the program and never complained about the products or the program until that time We, at Slim4Life, want all of our clients to be successfulThat is why a significant amount of our new clients every week are client referrals, just like Ms*** was referred by her friend Ms***They have a friend, relative or co-worker on our SooS I am rejecting this response because: after calling the number given to me I was told it would be 1-2 weeks. Fore they would return my call to even begin discussing what needs to happen. They are constantly giving me the run around.

Dear Revdex.com,                                                 Our mid cities area supervisor met with Ms. [redacted] last week. We explained to Ms. [redacted] that a list...

showing all the foods she would be eating on her program were shown to her before she enrolled in the program and signed her contract. Ms. [redacted] stated that this was true but she can not prepare her food like she normally does so she would be satisfied. Ms. [redacted] applied for a refund and our supervisor will bring her chart in to our business office on Tuesday for review by our board of directors. Ms. [redacted] and The Revdex.com will be informed of their decision. We as a company want to be fair to all clients. We live and work in this community and have been in business for over 30 years helping individuals lose weight. We are sending out a direct mail piece in September. Our client [redacted] is on the front of this mailer. [redacted] lost 95 pounds 2 years ago and she is a client in Hurst Texas. On the back of the mailer is [redacted] who lost 33 pounds over a year ago. Both ladies still have their weight off. Her friend [redacted] told her about Slim4Life. Ms. [redacted] is a client in Lakeworth. Ms. [redacted] referred Ms. [redacted] to our program. We then proceeded with service. We took a complete medical history on Ms. [redacted] which she signed (copy enclosed), when the counselor asked MS. [redacted] if she had any allergies, she said no. We explained the preparation phase of our program to Ms. [redacted]. We took Ms. [redacted]'s blood pressure, which was 130/84, and we took Ms. [redacted]'s measurements, which were chest: 53.5, waist 53, hip: 57, thigh: 24 and right arm: 18.5 inches. Ms. [redacted] picked out her starter supply of products, she signed for and left with $319.00 in products. I have included a copy of Ms. [redacted]'s Disclosure Statement which is signed. On 01/31/2017 Ms. [redacted] purchased $4322.10 in additional products. Ms. [redacted] was given a 30% discount on all her products for buying her products in volume purchase for her entire program. Ms. [redacted] came in for a counseling session on 02/02/2017, 02/03/2017, 02/04/2017, 02/07/2017, 02/11/2017, 02/15/2017, 02/16/2017, 02/20/2017, 02/23/2017, 02/24/2017, 02/25/2017, 02/27/2017, 03/01/2017, 03/03/2017, 03/09/2017 and 03/15/2017. On 03/15/2017 Ms. [redacted] told her counselor she was unhappy because she was allergic to soy, the counselor showed Ms. [redacted] her consultation sheet she filled out at the time of her consultation and where it asked about allergies she wrote codeine and did not write soy (copy enclosed). On 03/15/2017 Ms. [redacted] had lost a total of 16.4 pounds. Ms. [redacted] had received 18 counseling sessions In closing, Ms, [redacted] signed a contract which plainly states our refund policy. (TERMS AND CONDITIONS, RELEASE OF RESPONSIBILITY AND PERMISSION TO PROVIDE SERVICE) NON-REFUNDABLE. ALL NEW PROGRAMS INCLUDE ENROLLMENT AND SET-UP FEE OF $300.00 WHICH IS NON-REFUNDABLE. I also understand that ALL SALES ARE FINAL and that any and all Slim Snacks and nutritional products are non-refundable and non-exchangeable once the Slim Snacks and nutritional products leave the center. I also understand once given my assigned nutritional program at the program explanation class, the program becomes NONREFUNDABLE IN ITS ENTIRETY. I further understand that Slim4Life Weight Loss Centers are agreeing to provide me individual counseling up to 6 visits per week during my program for the contracted weeks set forth in this agreement during the designated business hours. I hereby grant permission to employees at Slim4Life Weight Loss Centers to provide me with weight loss services. I warrant to Slim4Life that I am in good health and fully able to participate in this program and do not require my Physician's release to do so. I agree to hold the company, its offices, nurses, and employees completely harmless from any and all claims, liabilities, or damages for personal injuries which I may suffer directly, or indirectly, from and participation in the program. The Manager of Hurst location said at one time Ms. [redacted] had said she was going to sell the products to someone she knows. We know we tried to work with Ms. [redacted] and Ms. [redacted] lost over 16 pounds in 5 weeks on the program and never complained about the products or the program until that time We, at Slim4Life, want all of our clients to be successful. That is why a significant amount of our new clients every week are client referrals, just like Ms. [redacted] was referred by her friend Ms. [redacted]. They have a friend, relative or co-worker on our 126 SooS program. We would love to help Ms. [redacted] achieve her goal weight and keep a healthy weight ongoing Ms. [redacted] can call us at ###-###-####, our corporate office, and we will be happy to set up an appointment with our Service Supervisor to restart her program.
