Review: I contracted with [redacted] DDS to provide me with an upper bridge. Work began 9/6/13.I paid him $29,000.00 for the bridge. He was a Revdex.com Member.It is now 3/19/2014. Dr. [redacted] has closed both offices, and the telephones have been disconnected.The temporary bridge that he gave me broke in several places. I also have medical conditions that require that I be able to chew my food thoroughly.I had to go to another dentist to have the temporary bridge repaired ($317.00). In February 2014, it became apparent that Dr. [redacted] was no where to be found. I had to start the complete process over with another dentist at a cost of $22,837.00.Dr. [redacted] had been working on the teeth since 09/06/13 and the bridge was not completed. The new dentist was able to produce an award winning smile in less than a month.Desired Settlement: $25,000.00 cash for goods paid for but not received.

Review: My 17 year old son [redacted] was seeing this Dentist for a year and a half. He received ortho care for his braces on a monthly basis. As of Jan. 2014 this dentist started cancelling all apts. and not responding to any emails. His phone was disconnected. My sons braces were to be removed this coming September. He was suppose to be having monthly braces adjustments till then and then he would have his braces removed and receive his retainer. All of which I have already paid for in full. My insurance had already paid out there portion in full as well. We finally drove down to the office to find it locked up with mail piled up on the floor. The rental property manager cant tell us where he has gone either. He has simply skipped town and left all his patients high and dry. I called the police to file a police report & they said I had to contact the Revdex.com. I have since taken my son to another Ortho to resume his treatment where I will have to pay out of pocket for services I have alreay paid for to Smart Dental. I want [redacted] license taken away from him so he cant do this to anybody else.Desired Settlement: I would like a refund for the rest of the services my son did not receive. That would be 9 months of braces adjustments and the braces removal & retainer which I paid for in advance in full. I would like [redacted] I License taken away so he cant do this to anyone else.
