1. First let me address the hours of operation. Our hours have not changed since January of of 2014 they are as follows,
Mon 8-12, 2:30-6
Tues 2:30-6
Wed 8-12
Thurs...
8-12, 2:30-6
Fri Closed
Sat Closed
Sun Closed
2. Ms. [redacted] has been a patient for a long time starting 3/28/14 coming 32 times from this date to 8/5/2015 which was her last date of service. Ms. [redacted] did call our office claiming our hours had changed after she came by the office unscheduled one evening close to 6:00 pm our closing time. We only take patients on a schedule not walk in. We called Ms. [redacted] the next morning after receiving her VM and let her know that our hour were the same and that if she wanted to schedule we would be happy to see her. We normally leave after we finish with our last patient for the day which typically means 5:45 had Ms. [redacted] called and let the office know she was planning on coming in I would have gladly waited on her.
3. After her second call requesting to cancel we calculated her unpaid balance and informed her we would cancel her auto draft that was originally signed 3/31/2015 after her final payment of $31.20 for already received services was paid on 8/15/15. Per Ms. [redacted]'s contract and agreement (see attached) her auto draft would fully process by 8/5/15. On 8/5 her payment was declined (see attached) which resulted in the $50 dollar late fee per our office policy's which are covered with each patient and posted at the front desk (see attached photo). We then waited 5 days and drafted Ms. [redacted]'s account on 8/10 for $81.20 and mailed her a paid in full receipt.
