Patient had missing teeth at her lower right(LR) quadrant of her mouth and was chosen to start first for less procedures involved and with ease to restore patient`s chewing function and molars support comparing more missing teeth on her left side of her mouth with upper left (UL)(there is sinus issue) and lower left (LL) quadrants involvedThis point was fully addressed to patient and consents signed by Patient.Patient had LR missing teeth #29,30, to be implantedBut at #area, she had no thick enough bone to accommodate a minimal size of implant, around #area, there was a big void inside her jawThe implant was places at her #or area is most reasonable site could be found and located, A bridge was planned to quickly restore her LR missing teethThis point was also fully addressed and communicated with Patient before the procedure .It made TWO visits not ONE to have two implants placedThe first one #was placed on 02/19/2016, the 2nd one #was placed on 03/16/One week later on 03/22/Patient had tissue cut over #area by her chewing hard food and she had to walk in our office to get help to stop bleeding Tissue around two implants all healed or integrated well after weeks and months of placementsThese can be proved by all X-rays taken during follvisits.Patient got her digital X-rays selected per her request via her email on 09/19/2016, CT Scan disks ( on 09/20/2016) and all Treatment Plans and Consent SIGNED BY HER picked up from officeInformation of brand and size of the implants was given to another dental office per Patient request over the phone with last digits as ***.Patient paid $leaving balance $on 02/19/at #implant placement , and paid $only leaving balance $on 03/16/at #implant placementPatient paid off her balance $until 08/19/when she came to office and requested modification of her original treatment and financial arrangement of total cost $13,settled at the beginning because her financial situation changed Patient disagreed to placement of two more implants at her UL and LL sides for only $which was all she could pay as she stated (all abutments and crowns were offered for free).These are all about it.
