Review: Please be advised as I have overpaid for Services provided on the 10/19/2012. First, I was told the incorrect amount for the treatment which involved upper back tooth. Extraction would have cost less than 100.00 vs almost a 1000.00. I was told it was going cost 160.00 in which I paid 100.00 on that day. I received a bill in the mail for over 900.00 for the procedure. I was very upset. It took time to try and pay the bill down, in the meantime my mouth suffered due to refusal of service. I have paid the office a total of $650.00 out of pocket. After consistant refusal of service, I called [redacted] to help with the billing issue. [redacted] discovered the incorrect codes and lack of documents to support the claim for my night guard. The office refuse to allow service until I paid half which was dec 2013. After that I requested an appointment and was denied again. The balance remaining was for my night guard in which I was told by 3 staff members were not in the office(never made). Once I requested to remove the guard, the guards magically appeared in the office. Corrections have been made and the balance below is my out of pocket cost. After repeated calls, the dental office submitted proper back up for the guards. I still have not receive any communications back. I have a new dentist now and the work that needs to be done now is costly.Desired Settlement: I'm requesting overpayment and im also requesting that the smile bright pay for work that has to be done due to refusal of service for a balance I did not owe.
