Numerous billing statements have been received from this business, even though many, many attempts have been made by me explaining and assisting the business with regard to my dental insurance. All billing statements have been with regard to DOS January 16, 2017. It is notable to indicate that the very first billing statement received by me from business was May of 2018 for this DOS January 16, 2017! Numerous phone calls have been made by me to the business and my insurance provider, as well as a letter to the business which details my efforts to resolve. Jessica has been my point of contact with this Smile Design Dentistry. Most recent billing statement from business to me is dated 11/06/18. My dental insurer actually paid this indicated billing statement on February 8, 2017, but Smile Design returned the check to my insurer.
Other (requires explanation) Desire that Smile Dentistry stop billing me. This 2017 DOS in question was paid by my insurance provider, and Smile Dentistry returned the check to my insurance company. Then, in 2018, they began to bill me on a regular basis for this DOS. No matter my attempts to assist the billing department, they have continued to harass me with these bills. I request that they NEVER bill me again. The bill in question for one visit, and we have never returned to that place of business.
Received email statement indicating a zero balance. At this time, the case can be closed. Thank you.
