About October 2017, I was told that I needed to correct my crowded, lower front teeth. I was told that Invisalign was a good option and that I would only need to be placed on 10-week trays. The cost was $4,000. I was reluctant but decided to proceed since it was a short period of time.

I got my trays, completed 10 weeks. My teeth were still crowded. No visible change. I was told that I could do some adjustment trays. Skeptical and reluctantly, I agreed to another 10 weeks as they suggested.

Got the second set of trays, completed 4 additional weeks (now 14 weeks in). During a check-up, I questioned the dental hygienist about the lack of visual change in my teeth. I asked her what my week 20 trays looked like. She pulled them out, and the trays didn't show that my teeth would be straight. She had a pulled look on her face. I asked her "how would my teeth be straight if the trays are straight'!" After some noticeable confusion, I was told that my trays were made from the incorrect mold and that I would need to redo them. Asked the dentist if the next round would straighten my teeth as I was told initially. She didn't provide a firm answer.

After doing research on Invisalign, I discovered that the average adult treatment is approximately 9-15 months, which is NOT what Smile Designs told me. I would not have agreed to this had they been upfront and honest with my treatment plan. Frustrated and feeling that I was dupped, I asked that they guarantee me that the third round of trays would straighten my teeth as they initially told me or provide me with a refund. They couldn't guarantee it so I asked for a refund.

Since March 26, 2018, I have been told numerous times that I will receive a refund. To date, I have received nothing. I have called and emailed Smile Designs multiple times with no response.

Had issues with product and services for Invisalign. Stopped treatment, had my credit card refunded. Now they are sending me invoices directly.
Had issues with product and services for Invisalign. I was told that it would be a 10-12 week treatment correct my teeth. Had a botched second treatment. When I complained, I asked how long the treatment will be and they could not provide me with an answer. researching on the internet, the average treatment time is 12 months for an adult. That is not what I was told. Because of that and the botched second treatment I requested a refund. Contacted the credit card company that the dentist office finance and received a full refund. Now the dentist office is sending me past due bills for $3,671.00 claiming that it is money I owe. I contest the charges for false advertising and botched second treatment. I have excellent credit, and this will have a negative impact.

First, thank you for confirming that you "messed up on one of the trays." However, for clarification, you messed up one "set" of trays. There are 10 trays in a set. I had a total of two sets made, one of which was botched as you confirmed. I find it difficult to do the math on the discount of $250 verses what I'm being billed, and what I endured because of incompetency.

I completed one set of trays. I went for my follow-up, and the dentist asked me if "I was happy with the results" after the first set, week 10? Seriously, my teeth are still crooked? I diligently followed the treatment plan. I did my part. I was told that I needed a second set.
I got impressions made for second set, which is technically a "refinement." I completed week 4 of the second set. During my next check-up appointment, I asked to see my liner for week 10. I asked because I was seeing little to no change in my teeth. The dental assistant showed me the tray for week 10. I asked why it was crooked? If my liner is crooked, then my teeth would be crooked at the completion of the treatment. She didn't have a response. I was then advised that they messed up my trays, and I would have to get a new imprint of my teeth and begin the process again. Upset by their mistake, incompetency, and misleading info on the treatment plan (it isn't a 10 to 12-weeks as I was led to believe), and because the dentist was unable to tell me when my teeth would be corrected I declined additional refinements. Based on my research, the average adult will need to complete about 12 months of treatment. Had I been told that I would never have agreed to do this!

In your response, you state that I "went through my entire treatment and was not happy with the end result." You are ABSOLUTELY CORRECT! My teeth are still crooked! You did offer "refinement" treatment, and I agreed to one refinement, but you botched it. How many refinements will I have to endure? You can't answer that, but you say that I completed my treatment plan?!
There is "clinical fault" on Smile Design; botched procedure which is a fault! Additionally, fault of false advertising, misleading patients, and you did not provide what you stated will be delivered but you ask me to pay?!

I was told that I would receive a refund. Not the measly $250 discount. I was getting the run around. After numerous attempts to resolve this, I contacted Credit Care to dispute the charge. I told them of my situation, and they credited my account. I find it hard to believe that a credit card company would relieve me of $4,000 unless there is fault. Take this off my ledger, credit my account or I will get my attorney on this matter and seek a class action lawsuit. If this affects my excellent credit rating, I will pursue this through the FCRA.

Bad business practice. The crown was installed without a pin and fell off in the first 3 month. The office refused to fix the problem. I was forced to go and redo the work somewhere else and pay for it again.

Due to undesirable patient interaction, *** was dismissed from the pracatice 1/9/18 and has been refunded for treatment.

Smile Design Dentistry, West Chase location provided horrible service. On 12/18/ 017 Dr H. performed root canal treatment, followed by porcelain crown installed on 1/5/018. The porcelain crown was installed without any pins and failed (fell off) within first 3 month. Office refuses to take responsibility for providing sloppy. failed job. The office staff is very rude, insulting patients in front of other customers sitting in the front lobby. I went to a different dentist to redo Dr H. job and brought the broken off crown with me. They could not believe that licensed professional would put in crown, knowing it would not stay! I can't afford to pay double for dentist's mistakes!

Insurance wont pay because of unjustified work
Went into this dentistry to have my teeth cleaned. The dentist advised that I cannot get a basic cleaning because of other issues. I was told I needed a deep cleaning, and fill first. The receptionist searched my insurance and advised everything will be covered, I would need to cover my delectable. I paid the delectable. Weeks later I was issued a failure to pay notice. I contacted my insurance who said all procedures allegedly preformed will be covered under my policy once the Dentist provides proof that the procedures was done, was necessary and preformed to standards. I contacted the dentist who said they were understaffed and will send the information to the insurance that day. I just received a final notice to pay or be sent to collections. I called the dentist and it appears they don't have proof that the procedures were valid and the insurance wont pay because of this. I contacted the insurance company and they are now feeling this maybe insurance fraud.

provide information to insurance. Adjust the billing and provide a zero balance on m behalf.

As with all insurance companies, they only provide an estimate of what they may pay until the claim is received/reviewed. This is also stated within our office financial policy.

Upon receipt of the initial denial last year, we submitted an appeal to *** and have not received payment. We have since followed up a third time with the insurance company as recent as last week, and they had not yet reviewed the appeal we sent on the patient's behalf from when we sent it last year. We escalated the appeal request and have been provided with a case #XXXXXXX and are to receive a response within 30 days.

Any unpaid insurance portion can be disputed by the patient with their insurance company, as the policy agreement is between the patient and their insurance company.

As with all accounts, we continue to bill the patient until the payment is received, and by doing this, it also alerts the patient that their insurance company has not paid us and they can then dispute as well. However, when the patient works to aid in payment from the insurance company and supports our efforts, we will not turn the account over to collections unless we can not come to an agreement as to their unpaid balance. Simply communicating that we provided a treatment plan showing what the insurance will pay does not warrant active resolution by the patient.

To date, the account has been noted as "in the appeal process". The patient will continue to receive statements until the balance has been paid. The patient may contact their insurance company at any time to check on the status of the appeal.

Spoke with the insurance company, they advised that the dentist did not provided sufficient proof that the procedure was needed. If sufficient proof is provided they will cover the cost. I will follow up with the insurance once again.

*** has already done the first appeal and the dentist office did not include the periodontal charting and the x-rays in the appeal, only notes, if the dentist office had included all required documents, like they did initially, a second Principal dental consultant would have reviewed. I know, I know it is crazy but that is how the appeal process is setup.

At this point a 2nd appeal must take place where once again the periodontal charting, the x-rays, the clinical notes and a narrative from Dr. H. needs to be included. If the 2nd appeal does not result in the services being covered THEN a phone call between Dr. H. and Principal can be arranged to discuss each of their findings as to why it should or should not be covered. The dentist office is a preferred provider and does know what the procedures are for appealing a denial.

Also you need to let the dentist office know they can make a written request of the guidelines to substantiate periodontal disease and what periodontal services are covered.

As per our last response, we will look at the account again once the insurance company provides a final answer to our appeal in black and white. However, this does not guarantee any unpaid balance will be written off. To date, we do not have any response from the insurance company.

Additionally, due to the patients reported findings during the Comprehensive Oral Evaluation, along with xrays and periodontal probing, the procedures performed are more than warranted due to his oral health at the time of service.

The insurance company provided Ref# XXXXXXX is the patient would like to contact them. Again, once we receive a response from the insurance in black and white, we can proceed as needed.

went to smile designs in nov they said there denture repairman could fix my denture charged me $235.00 plus whatever fl blue policy paid them worked approx. 90 days then rebroke took it back they sent it back out this time the fix lasted under 7 days head dental moniter has not called me back when I left message and don't want to warrenety there work there the ones that said it could be fixed to begin with

On 11/29/17, Mr. was advised he needed a new partial as the one he came in with was a "healing/temp" partial including 2 missing teeth, however stating he only wanted repaired, would maybe do one "next year". Mr. was advised while we would send for repair, we do not warranty such repairs. With that being said, Mr. was given a treatment plan for the repair, requesting only the repair.

they did refund my $235.00 to my credit card I guess they did not want to meet Jackie c. of ch 28 thanks Jackie or be on tv and when they first did it they did not tell me no warrenety they said there guy could do it on 2 nd visit they said no warrenety issue was brought up so they lied to me m and you thanks and ill tell people to go else where for dental work if you do work you have to back it up

Fraudulent practices, frequent staff overturn and charging for services NOT
rendered. Their gross negligence is off the charts. Save yourself and go somewhere else and I mean anywhere else.

I have been explaining for over a year to no avail. It is simple I paid for services that were never rendered. (Due to the frequent staff turnover) when I came in to have my onlay put on (that I had already paid for) there was yet another temporary dentist there that flat out refused to apply an onlay that she herself did not have made. I was flabbergasted at the neglegent practice. Then I was told to wait for the new permanent dentist. I checked back a week latter and the permanent dentist position was not filled yet. I explained that I cannot wait because the temporary onlay had fallen out and I needed to have this addressed right away. The reply was that my credit card and insurance would be refunded. I was put in a desperate and inconceivable situation and found a new dentist that took on this issue. Because of the neglect I suffered this simple onlay turned into the very same tooth needing a crown. It was a horrible mismanagement of a patients needs. Then 14 months ago (a little over a year after the neglect I suffered by the staff at the time my onlay placement by "Smile Design of Westchase") I contacted my dental insurance to ask about my current coverage and inquired about the refund from the onlay that smile design refused to complete. I wanted to know what my total remaining coverage was with the refunded amount. Met life informed me that no refund was ever given to them by smile design and they urged me to contact them as they couldn't see any reason as to why a dentist would knowingly charge for services never rendered and that perhaps they neglected to do so and just need a reminder. Well after checking that my Credit card was never refunded either I contacted Smile design and explained the situation fully expecting them to refund me. However it has been 14 months of supposed research on their count (that I now see was nothing more then a run around. I contacted Smile design at least one a month since until I was finally told that they don't feel they owe me a refund. Again this is complete negligence and fraud. How can a dental practice refuse a total refund for services that the practice flat out refused to render? That is fraudulent and unwarranted period.

The business charged us without considering our insurance. We requested the refund almost 4 weeks ago and has not received it yet.
My husband had a dentist appointment on 10/31/17. He was charged the full amount instead of them charging our insurance. They had used my insurance the week prior without problem for the exact same procedures. I contacted them on 11/3/17 via their website. I got an email back on 11/6/17. My husband then contacted them and provided the requested information (card number, exp date, cvs) the same day. I contacted them again on 11/21/17 since it had been more than 2 weeks and they had told us it would be "a couple of weeks". I was told later the same day that they would check on it and let me know the next day. They got back to me on 11/24/17 and said they were working on it and would get back to me on 11/28/17. I have not heard from them again.

I want the $174 they owe us refunded.

Mr *** came in for appointment and paid for what we anticipated his portion to be. A day or later his wife called asking about the amount he paid. Upon rechecking with insurance, Mr *** was overcharged fro visit. Gave patient a refund of amount knew was an overcharge and explained once insurance pays will refund any remaining over-payment. To date, Mr *** does not have any credit on account- still fighting with insurance to pay and if insurance pays anything else, it will be refunded to patient. I understand the patients frustration unfortunately the insurance company has not paid anymore on claim as of yet.

Our insurance discount plan was very specific what the service fee was for a dental cleaning. They charged me one sum one week, then on the exact same plan charged my husband significantly more. We have not gotten a bill for the remaining bill from them for my dental cleaning so clearly the insurance company has managed to get that charge taken care of. So without written evidence of what is going on, I do not accept their explanation. Given our initial dealings with them after we first contacted them, I have no faith that they are being honest about what is going on.

I was quoted a rate for 2 crowns. I paid what was quoted. When the work was completed they asked for an additional $255.80. That should be refunded
I contacted *** Dental who informed me that I was not supposed to pay that as it had already been paid. A letter was sent to the Dental office from *** Dental informing them that I should be refunded for overpayment. The Dental office has not refunded me or responded.

Desired Outcome

I want my refund that is owed to me. You do not quote one price then because you did not get what you wanted from *** Dental, you then charge me. They gave me a quote of $730.80 and I ended up paying $773.00. I did not gripe about that, but paid it. Then they charge an additional $255.80 that was a duplicate payment. I want my refund owed in the amount of $255.80

If it is found upon re-audit that a refund was previously credited to the patient's credit card, we will take this money back.

I received my first response from the *** office yesterday, 11/1/2017. I gave them my credit card information to credit the card back. I do not see a credit yet. So, I am not sure how they say that they paid me back the money. Their "audit" will find that they never did. I will give it one more day and if the credit is not applied will take it to small claims. Thank you for your help.
