On Dec 19, 2019 I had my first and last visit at Sand Lake Dentistry office. The Dr told me the only course of action is to have all the teeth removed except 1 or 2 then partials. I paid $630.38. He worked on a tooth then I had measurements for partials.The pain was worst and worst and after 2 weeks I called the office for a prescription. Soraya the Office Mng said she cant help , the Dr was in vacation and take instead 4 over the counter doze of Ibuprofen as needed Because I refused she told me to contact a different location for the prescription who also refused for good reasons.Took 9 phone calls that day to finally get the medication. I told myself I cant trust this office for such a procedure if I had these problems getting a prescription. I called 10 days before appointed day to cancel everything.Plenty time for the lab to stop making the partials. Soraya tried to convince me to go back and that she will have her Mng call me .I told her even if I wouldn't be charged anything for any future procedures I will not go back there. I had to go to a different office and have that fixed. The dr Put a temp crown over an infected tooth that he failed to see. Another Dr had to fix the mistakes.When I called again about the refund for what I paid but was not delivered her attitude was changed. Dismissive, arrogant, patronizing. This time she refused to give me her supervisor contact information even though first time she volunteered to do that.I kept receiving instead pile after pile of claims , with different amounts each time for the same alleged done procedures. I showed my Dr the claims and he said besides being overpriced none of the procedures were done.She never contacted me back like she said.She ignored not only me but My Insurance provider too when they requested proof of the work done.As requested I provided My insurance with proof of payment for what I was supposed to have done also a claim with the second dr notes stating none of the claimed work was done. 7 months of phone calls .

Refund I want a refund for $602.My Insurance provider requests were ignored as well so they sent a letter in my favor. Dozens of phone calls and she was saying the same thing : I have to pay for partials because they were ready. Never seen them, or used them and 10 days was enough time to cancel.On a 3 way conversation she said to my insurance cust. Rep she will give me a refund but the partials are there.Instead I got a $700 bill.Dozens of calls and aggravation and the refund was an insulting $23.

I accepted the treatment when I was presented with the treatment case paper .(See attachment ) If they checked with my insurance or not I dont know .Most likely not because all I know is that my insurance denied almost everything they claimed they did . 3 attachments shows claims with different amounts .
Is not my job to check with the insurance but the office's . I attached the second dr claim that shows what he has done . It took him 1 hour and 15 min for the root canal and build up . Sand Lake dr spent like 15 minutes with me . Impossible to do what the office claimed he did in just 15 min. Besides everyone knows how a root canal and build up feels like..You know if a dentist drills a hole in your tooth or not . And when I asked why they dont do a root canal before putting the temporary crown the assistant told me they don't do that because the tooth die . If I would be 20 I would understand to keep it alive longer . Is not my case . So , they didnt check with my insurance first and they didn't do what they claimed they did . I attached one of Sand lake claims with the second Dr hand notes on it stating almost nothing was done but they claimed the payment for it . Overpriced as well . He said if there are any questions he can be contacted at any time
Also is not fair to pay for a bad work . For 2 weeks I was in agony . I had to go to an emergency office. The dr told me I didn't had a nerve there , was just ,,a bag of puss "So dr K... either didn't see if an infection was there (I was not in any pain when I went there ) or the tooth got infected after his work . Either way that is not good . But I accept errors happens . What I dont accept is being ignored and patronized .
On June 8 th on conference call Soraya is true , told me I am still responsible for the lab fees but that because she said the partials were already there in the office . There were made so I have to pay . Again I canceled any treatment at that office after the first and last visit on Jan 8th 2020 8 days before the next appointment scheduled on Jan 16th ( So 8 days in advance ) According to the document she attached here the partials were not made untill June 18 th . So they were NOT ready like she said starting from Jan 8th when she refused to consider a refund . So according to their own documentation those partials were not done until june 18th , 6 months AFTER I i canceled the appointment . Every time I called starting with january 8 th till june she said the same thing ,, The partials are there " Almost every call to a business advice the caller that the call might be recorded for quality and training purposes . Soraya talked in two occasions with the insurace agents saying the same thing over and over. The partials are there . So is not just my word but those who pay part of bills as well . Plus the documentation I attached with contradicting statements and figures . I have no need for the partials . There are 2 reasons I said I want them :
1 . If you pay for something then is yours , otherwise is fraud
Second , I just wanted to prove that I KNOW they are not ready . And I was right . As a last attempt to have her admit the truth on June 18 th I told her I want her to send me the partials . This time she said she have to check with the lab (so the partials were not at the office anymore . ? ) and she will let me know . After 7 months dealing with that office I knew what ,, I will let you know "means . Means never . I pressed and asked WHEN she think she will have an answer . Lilke always she didnt know , probably 5-10 days . I agreed telling her I hope to hear back in that time frame And I also said if she doesn't call then I WILL . (Why somebody need 10 days to make a phone call ... only she knows ) And I told her I will give her not 10 days but 2 weeks to hear back . Of course she didnt call back , as I repeatedly said , she never did , so I called and left a message to an assistant to call me back . Was Thursday morning . She didnt call that day or the whole day Friday . By that time I had enough . My goodwill to accept that behavior was treated as stupidity or weakness . So Friday evening I wrote a review based on facts and documentation . Monday at 7.30am she called demanding me to take the review down . Saying that she called but nobody answered . I told her the phone longs dont lie , nobody called me .She was again having an attitude asking ,, Oh , so just because I didnt call right away you write a review ?" In my book right away is 5-10 minutes not 2 whole days . Besides 6 months is more than enough time to fix a situation (Offices were closed just a month due to covid
)In the end she admited she didnt call by saying ,, What difference does it make if I did or didn't call last week ? I called now "
Nothing left to be said but No comment .
She was unhappy because I made her responsible for the whole situation . If she chose to withheld her supervisor contact infos by her own volition or by requirement I dont know but whoever is responsible for all this situation showed little professionalism to say the least . On their business website says I will be contacted by a supervisor . I wasn't , I don't expect to , and I don't want to anymore . I know one thing : a good Mng helps an employee in a difficult situation . The fact that the supervisor decided to ignore everything tells a lot about that office starting at the very top .
I said I will pay for what was donne , no matter how bad and how much pain I had to endure . But I WILL NOT pay for what was NOT done . Just because the lab did the work 6 months AFTER cancelation is not my problem . I was charged $630 . I refuse to pay $350 for partials made 6 months AFTER she said they were made and ready and for a never done build up . I attached a letter from the insurance having the case favorable to me regarding the refund . The reason was the office didn't provide documentation for their claims when was required . So again confirms the lack of response from this office not only to me but to the insurance agency as well .
The only thing I accept is a refund for partials and build up . $474-$23 =$451
Nothing else is acceptable.

Your complaint was escalated to upper management and our Compliance Manager after stating you were not satisfied with the initial resolution.

After reviewing the details of your complaint & rebuttal to the initial resolution provided for this matter, the decision was made to refund you an additional amount of $652.38 in good faith.

With this additional amount, you are receiving a refund for the full amount you paid for treatment on 12/19/2019.

We appreciate you patience and feedback with this matter.

Dental office has over charged my wife under our policy. Attached is the proof of this.
I discovered this issue 3 weeks ago when I called my insurance company to discuss the bills we paid to the dental office. The attached documents show that we should have only paid 210.80 on 1/24/19. We paid 592.80, which is 382.00 more than we should have paid. She returned on 1/28/19 to finish the services and paid 532.20. We should have only paid 204.20 for the services rendered, which is 328.00 more than we should have. The attached documents came from my insurance company and they called the dentist office to help me resolve this issue. The dentist office said to give them a week and they would look into it. I waited and it has now been 2 weeks. I called today and they informed me the refund would only be 66.40 after the insurance company informed them the refund should be 710.00. When I went to this dentist office a man outside told me to go elsewhere as they are crooks. I thought he was just crazy, but now I wish that I would have listened to his rants.

Desired Outcome

I want to have a refund of the total amount owed to us ASAP. The amount is 710.00 USD, which has been delayed for over 3 weeks. I want an investigation launched into their bill department as they must have done this to others. They have been fighting my insurance even though the violated the policy and the insurance company explained this and sent them the contract they signed to support it. They need to be audited as this has to be more than just me. We live in an area where the cost of living is very high in our area compared to other parts of Orlando outside of downtown. Bill of this size are part of the norm for the area, so most people would not look into it. This office needs to be looked at as they are most likely overcharging their patients. They give a lot of trouble probably in the hopes that you will give up.

January 28,2019. 'These particular procedure is not covered by the Insurance company. Mr.
*** called our ofce several times and we advised him that he will he receiving his refund
By August 6 2019.

It appears this company. Smile Design Dentistry, has an issue with their billing practices. I want to speak with a finance leader in the company regarding their billing practices and the fact that my insurance contracted rates were not honored. I was charged over $600 for a pulled tooth. This office overcharged my credit card and needs to send me a refund rather than me give them my credit card information again. I do not trust them or the lady doing their billing. I have contacted them on numerous occasions including going into the office to discuss the bill, They have returned my call asking my daughter for my CC number. They told me that they accepted my insurance for their services, when in fact they did not. I paid them over $2,100, they billed the insurance over $3,000. The paperwork from the insurance to me states that they denied the claims because they need more information from the dentist. It is useless to speak with the lady in charge of billing in this office as her responses make no sense. I want to review my entire case with someone else in this company. not anyone located at the Sand Lake office. Patient was my adult special needs daughter.

Contact by the Business Email correspondence to discuss entire scenerio and then In person meeting to review the treatment received and the billing process, payments and explanantion of how they are going to resolve this issue for me. I do not want to talk with anyone at the Sand Lake office. I want to speak with someone higher up in the corporate ladder for resolution. This is supposedly a 'Family Owned' business.

Dear Consumer affairs representative,
we are responding back to the complaint placed by Mrs. Case ll ***.
On August 15 2018, patient Shannon Phillips and her mother Mrs. presented in
our office for a complete dental examination. Patient complained of broken teeth and were
interested in having a cleaning as well.'
Upon evaluating patient Shannon teeth, Dr. K diagnosed patient with moderate
inammation of the gums, patient had a buildup of plaque, stains, and generalized calculus
deposits therefore recommending gum theraphy along with an antibiotic that is placed
immediately after the gum theraphy ls performed inlthe gum tissue. .Dr. K, also
recommended Extraction of teeth #15 and Root canal crown of Tooth #30. Patient Shannon
and her mother requested an estimate for extraction of tooth number #30 and the gum '
theraphy with the antibiotic as well." Estimates for all the dental treatment was presented to
them. We explained and showed to the patlent's mom about the insurance and went over
what was covered and not covered. They both understood and consented the treatment
rendered. ' ,
They made anappointment to come in August 21, 2018 to start on the gum theraphy and have
tooth #15 taken out. On September 12, 2018 patient nished her gum theraphy and was
advised to come back every three months for periodontal maintenance. .
Mrs. ***and patient Shannon were explained that we will file all the treatment
done anyway to the insurance and if they will cover the exam and x-rays a refund will be given
to the patient.
On September 21,, 2019 we received an insurance check for the amount of $116.
We have called Mrs. to let her know about the refund several times.
However, she won't retum the, phone calls to us.
We are very sorry we could not meet her expectations. We wished we have not lost Shannon as
a patient in our pratice .
Enclosed, please find oopy of the x-rays taken and the intraoral pictures taken for the patient
and all the paper work supporting the Information pprovided.
