To whom it may concern:
This letter is in response to your letter of complaint, in which a patient questions our collection
processes and our charges. I am attaching a blank copy of our patient registration form that clearly
states our collection policy. This form was signed and dated by this...

patient.
There are no hidden charges, as the patient states. On the day of service,the patient paid his estimated
portion. We always state that this is an estimate and if there is a difference after the insurance pays
their portion, we will send a bill for the balance. That is also clearly stated in the attached patient
registration form. We did send the patient four statements, followed by a past due letter, a collection
letter, and a final letter stating that the account will go to a credit bureau if not paid within 15 days. I
believe that we went above the necessary in order to notify the patient of his financial responsibility.
None of the statements or letters was returned as undeliverable, so we had no reason to believe that
the patient did not receive them.
It is not our policy to make collection telephone calls, unless correspondence had been returned as
"undeliverable," which they were not. We have had no notification of any change of address. The
emails and texts that the patient mentions receiving are for appointment confirmation and are
completed by an independent contracting company and is not connected to our practice management.
We are certainly sorry for any miscommunication, and it is never our desire to send patients to the
credit bureau. Unfortunately, infrequently this does happen. When the patient called our office to
voice his displeasure, I stated the above and stated that ¡f he paid the account we would indeed remove
his account from the credit bureau and if done in a timely manner, would not affect his credit rating. He
did not pay.

The paint on my car was damaged during a car wash on 2/24/2014. At the start of the wash there are two men who wash the car with mops prior to you entering the automatic machine wash. One of these men banged the hard mop handle against the rear bumper causing a large and deep chip in the paint. I informed the [redacted] on duty of the problem, [redacted] who advised I come back the next morning to speak to the [redacted], [redacted]. The next morning I went back at the time told by [redacted] and spoke to the [redacted].The [redacted] told me there was no way a mop handle could have caused the damage.I found that surprising since the chip is crescent shaped and would match the side of a mop handle. The paint chip is fairly deep and goes into the clearcoat underlying the paint. The [redacted] did acknowledge that much.Product_Or_Service: exterior car washDesired SettlementI would like the damage to my car fixed and paid for by the car wash that caused the damage.Business Response On 2-24-14 I received a phone call from the [redacted] on duty, that a customer called over the telephone to report damage to his rear bumper.The manager on duty asked the customer if he could come in on 2-25-14 so I could look at the damage,I was out of town on 2-24-14. On 2-25-14 at 8:00 am, I met with the customer.The customer stated he went through our car wash and his bumper was damaged.The customer stated at the beginning of the car wash, he heard a loud bang.He stated that the attendant must have hit his bumper with the mop that we preclean the vehicles with.He than left our facility and went to work and noticed he had a scratch on his rear bumper. I asked the customer if he had talked to the manager or anyone at the facility about the damage or the "loud Bang".The customer stated he called over the phone to make the damage claim,he said there was no one there to talk to about the noise or the damage.I asked the customer if he looked at his vehicle after he exited the car wash, he said he did not. I explained to the customer we do not use mops to preclean the vehicle, We use hog hair brushes, which are very expensive&are guaranteed from the manufacturer not to scratch.He than stated the damage was done from the handle.I questioned him about why he did not talk to a manager about the "bang noise" or the damage, He said there was no one there.during our operation that day&mostly everyday we have 2 employees precleaning&vacuuming the vehicles, at the end of the car wash we have 2 employees towel drying the vehicle and 2 employees cleaning the interior windows,and the manager which works the cash register and collects the money....The customer paid the manager on that day, after he said he he heard a "big bang noise" and the attendant hit his vehicle with the prepstick, although never put in a damage report or looked at his vehicle on site to see if there could be any damage from "this loud bang" instead went to work and called at a later time. After looking at the damage on the vehicle, the gouge is inconsistent with the prep stick we use....the prep stick is a smoothe wodden broom stick handle.the damage is a gouge with a line in the middle of it...I asked the customer if I could show him the prep stick,so he had a better idea of what it exactly looked like, He stated he was a very intelligent well educated man and didn't need to see the prepstick that he knew it happened at our car wash..I also reassured the customer we have been in business 25+years, this is not how we do business.If I felt this damage occurred at our car wash I would reimburse him 100%, although I feel 100% confident this damage did not happen at our facility.The customer stated he was not happy with my decision, I told him I was sorry I couldn't help him.He got into his vehicle and drove away.Consumer Response (The consumer indicated he/she DID NOT accept the response from the business.)owner admits the"prep stick" has a wooden handle.A wooden handle can scratch a car especially if it hits a corner painted area at car rear. When I heard the bang I did not know they hit a painted area of the car. I assumed it was the bumper which has a protective guard.I was in a rush to return back to work on time which was only 5 minutes away.There I got out of my car to remove something from the trunk and noticed the damaged and called right away.there are no signs at the car wash telling how to report damage.I used a coupon from a previously purchased coupon book. coupon was given to the attendant at the start of the wash.there was nobody at the office, which is located at the end of the wash line, where you can usuasly see mgmt.due to glass windows of the office. I was never oferred a damgage report to fill out,not even by the employee who hit the car.The only thing I was asked to do was to come back 8am nextday and owner admits I did that.At that time I was given the runaround about how the wooden handle could not have done that. There are no other signs of damage to the rear of my car which could indicate damge caused by another way.ie, bumper dent from backing up into something at another time or location as the owner suggested when I met with him.I have photos of the car that I am happy to share and there may be video in the car wash of what happened that day.Is the owner willing to share the video as proof??So far it was not offered to me or the Revdex.com.I have the same position as the start of this complaint.I want the repair paid for.Also, the wash is named and advertised as Soft Touch car wash why is part of the washing being done with a "prep stick" with a wooden handle??? The least they could do would be to cover the wooden handle with rubber or something that could never scratch car paintFinal Business Response My reason for denying this damage,is for the fact that the damage is inconsistent,which was also offered to show the customer our prep brush and to try to re-enact how the brush could have caused such damage which the customer refused.(which the customer is still at anytime welcome to see our operation)The customer is absolutely 100% right that the same outcome would have been the same if the damage was brought to managements attention, due to the Gouge not being consistant with the prep stick(which again gouge has a line going through it) The point of stressing the customer left the business,without telling any attendants or management on duty,is very suspicious& only shows that the customer realized damage to his vehicle, the last place he was,was at the car wash...This doesn't mean it happened at the car wash...Once again 8 employees on dury, 2 employees prewashing the vehicle, after the customer Hears "a loud bang noise" says nothing to the other 5-6 employees or management at the other end of the car wash which had to towel dry his vehicle and take payment..but calls after getting to work and realizing there is damage to the back of the vehicle..There is no argument the customer called after arriving to work.as far as looking at the vehicle,after exiting the car wash,we have 2 parking lots on our propery and a very big side street behind the car wash where there is plenty of room to park and most customers do use our parking lots and the rear street to touch up their own vehicles,if they choose..I believe we are running around in circles here. We are refusing to pay any part of the damage to this customers vehicle, once again we do not feel any responsibility and do not feel this damage occurred at our location....Final Consumer Response (The consumer indicated he/she DID NOT accept the response from the business.)there was no resolution offered by the owner,only denials and excuses to try to resolve himself of the resposibility.I do not accept his answers and want the Revdex.com to proceed as they feel fit.Again, I am wiiling to show vrhicle or send photos and provide phone records as needed.
