Patient [redacted] was informed from the start the procedures that were done on her We started the root canal and crown prep and she was then supposed to return back in two weeks for us to finish the root canal and seat the permanent crown she has refused to do that I have spoken to her several times about this as well as her insurance company The reason she has a balance of $ is because her insurance pays us on the seat date of cementing the crown and she refuses to come in to have this taken care of Her out of pocket cost would be significantly less if she would just come in to have the crown seated We have left several messages as well as sent several statements but haven’t heard anything from [redacted] See Attached Documents!
