[redacted] Dear Ms [redacted] , This letter is in response to a complaint filed by [redacted] We at Song Periodontics and Dental Implants take pride in providing excellent periodontal and implant careWe provide individualized and customized care to all our patientsMs [redacted] first came to my office on 10/28/She was referred by her general dentist, Dr [redacted] for periodontal evaluationAt that appointment, we discussed her diagnosis, treatment plan and alternative optionsFinancial consultation was completed after her evaluation/examinationMy financial coordinator gave her an estimate of $co-payment for her recommended treatment(a signed copy enclosed- Exhibit A) The estimate is based on the information we obtained from her insurance plan, ***Our form states that it is an estimate and not a guarantee of payment Unfortunately, [redacted] group plan for open table processed our claim under medical rather than dentalThe explanation of benefits states that the claim was considered in accordance with the [redacted] rates (not a file we can access), paid @60% after deductibleFurthermore, it also said that additional benefits are not available under the dental plan (insurance that was provided to our office)As a result, Ms[redacted] estimated co-payment was a lot more than she expectedWe are a third party and we have no control over the individual policy that her employer, "Open Table" chooses to provide to their employeeAll other medical insurances require the dental "explanation of benefits" prior to processing claims which is not the case for Ms [redacted] 's planWe have recommended for Ms [redacted] to talk to her HR department about this very unusual practiceI have been more than generous by not: charging for tooth #(additional site) when I completed grafting for teeth #'s & I even offered free touch ups when she expressed that she was unhappy with the treatmentIn summary, I understand Ms [redacted] 's complaint and frustrations regarding the amount she had to payI truly wish that we can help her more but it's a matter outside of our controlIt's something that should be directed to her insurance company or her employerIt also is the responsibility of each insured to understand his/her plan including any limitations or exclusionsIf you have any questions or need additional information, please do not hesitate to call my officeI also respectfully request that this complaint be removed from our siteWe take our online business reputation seriouslyThank you for your prompt attention to this matter ?

came to my office on 10/28/2014. She was referred by her general dentist, Dr. [redacted] for periodontal evaluation. At that appointment, we discussed her diagnosis, treatment plan and alternative options. Financial consultation was completed after her evaluation/examination. My financial coordinator gave her an estimate of $651.00 co-payment for her recommended treatment. (a signed copy enclosed- Exhibit A) The estimate is based on the information we obtained from her insurance plan, [redacted]. Our form states that it is an estimate and not a guarantee of payment Unfortunately, [redacted] group plan for open table processed our claim under medical rather than dental. The explanation of benefits states that the claim was considered in accordance with the [redacted] rates (not a file we can access), paid @60% after deductible. Furthermore, it also said that additional benefits are not available under the dental plan (insurance that was provided to our office). All other medical insurances require the dental "explanation of benefits" prior to processing claims which is not the case for Ms. [redacted]'s plan. We have recommended for Ms. [redacted] to talk to her HR department about this very unusual practice. I have been more than generous by not: charging for tooth #4 (additional site) when I completed grafting for teeth #'s 3 & 5. I even offered free touch ups when she expressed that she was unhappy with the treatment. In summary, I understand Ms. [redacted]'s complaint and frustrations regarding the amount she had to pay. I truly wish that we can help her more but it's a matter outside of our control. It's something that should be directed to her insurance company or her employer. It also is the responsibility of each insured to understand his/her plan including any limitations or exclusions. If you have any questions or need additional information, please do not hesitate to call my office. I also respectfully request that this complaint be removed from our site.
