He had one hour remaining in his balance and he was offered the one hour meeting on 12/18/16, he declined He insisted on a refund, the disclosure that he signed clearly indicates "no refunds for unused hours." 1) this customer was provided an itemized billing statement evidencing his visits and time expended, reason for services on 12/16/Description has been blacked out for his privacy. 2) I am a private pay providerI do not participate with insurance panels, this customer is aware at this fact when he engaged servicesI do not have any obligation or contractual agreement to complete inspaperwork I offered admin service for such a special need as a fee for service. He refused.
