Review: On 6/8 my cap fell out of my teeth, I went to this dentist to put it back in on 6/11/15- I only wanted him to xray that part of my teeth & the Dr. [redacted] came in & stated he wants me to get a full xray of my teeth & I told him I came in Oct 2014 & you have that already, he said he needs to have full xray so he can come up w/ a plan & it will be no cost to me. I said ok. I was seated in the dentist chair & the dentist Dr. [redacted] came in & said he has to pull all of my top teeth out- I have BP issues & I am on 6 different BP meds and I started to get excited and started to cry. I told him that just can't have them pulled, he said ok let me look again & see what I can do for you. He came back & said well I can pull your top 4 front teeth and put a bridge I asked him how much that will be? He left & a guy came in & told me It would be around $10K but my dental plan will pay for $1500 &I can finance the rest & they will give me a 20% discnt. I told him I am on blood thinners the dentist came back in & said my doctor would have to approve me being off the blood thinnes for 5 days. I told him I want to wait until he approves it. (Note: he never approved it) He said ok let me prep your teeth so they can go in & get it done. I was so upset & afraid he said I will take care of you they guy came back in asked me to sign this & I did not have my glasses w/ me he showed me where to sign (I could not read it) on this glass thing. The denist sanded my teeth & put a plastic cap on. I asked for my original cap & the guy said he destroyed it. I was waiting to talk w/ my Neurosurgeon before I moved forward & on 6/16/15 I recieved an email from my Dental insur, that there was a claim for $6K submited I called the insur & they stated he sent a claim stating that he did procelain caps and bridges on 6/11/15 billed $6K he did not do any of that procedure, I had to send a grievance to the insur co, 6/27/15 I rec'd a bill for $6K from a loan co that I owe form dental work done 6/11/15.Desired Settlement: He never did any of what he billed my insurance or what he billed me fore. I would like him to adjust and rebill my insurance copay for xray and prep work. For the loan he needs to them them he did not do any work on me. I cancelled by fax all work on 6/15/15 due to the insur company told me he is billing for procedure and items that he did not do. They are sending me to another dentist for review of what they did and how much work they have to do. Please help I am 55 yrs old very ill and I cant deal with this scam he is doing to elderly patients. [redacted]

I am rejecting Business Response TO:Revdex.com[redacted]Hi [redacted],My response to their submittal:·1) They did not immediately reb ill the insurance company the correct amount. I hadto call and tell my insurance company that they were billing for work and suppliesthat were not done and was assigned an auditor her name was [redacted]. [redacted]faxed. So. Oceanside Dental requesting proof of work and supplies that they statedwere done on 6/11/15. She did not get a response so she had to call the dentaloffice and spoke with [redacted] and on 7/16/15 they did send a corrected claim toDelta Dental. See attached for the corrected claim. The point is they were billingme and my dental insurance supplies and procedures that had not taken place.I have contacted the Dental board association and filed a formal complaint. Belowis two in read of the seven lists of dental fraud. The original claim they sent toDelta Dental was for $5,826.00 the corrected claim was for $104.00.Acts of dental fraud contain three defining features: intent, deception, andunlawful gain.These scams can include, but are not limited to:• Billing for services not rendered• Misrepresenting dates of service• Waiving of deductibles and/or copayments• Misrepresentation of services• Unbundling services charges• Overcharging or upcoding routine services• Diagnosing unnecessary or incorrect treatment2) The dentist I am now seeing stated I do not need any front teeth pulled they arestrong and in tact and that the previous dentist ruined my teeth for life and for meto get it fixed I had to pay $13,000. I had to take money from my 401K plan topay for this.3) Like I said before they were having me sign and intial items that I could not seeand I told them several times I don't have my glasses and can't see and the dentaltech and [redacted] stated it's ok I am just signing for the doctor to review and lookat my teeth. I took their word for it trusting in them. I was told the costs and I toldlJ--1Ithem I don't want anything done until my primary doctor gives me the ok to havework done since I am taking blood thinners. They agreed verbally to that but keptwanting to make all these appointments that I would not do until my doctor gaveme the ok.4) On 6/23/15 I faxed over cancellation of all services because I found out that theywere billing my dental insurance without doing the work. I did not get a responseform them. (See attached fax confirmation)5) On 6/27/15 I received a $6,000 bill from a company called [redacted]s andI had no idea who that was. I called them and they stated So. Oceanside Dentalapplied for me a loan for $6,000 for procedures that were done on 6/11/15. I toldthem I never applied for anything and they told me to file a fraud complaint whichI did. They still show I owe the $6,000 and have not received anything from So.Oceanside Dental as of this date to issue a credit of $6,000.6) On 6/29/15 I faxed over a 2nd request for cancellation and added they billed mefor $6,000 from [redacted] and that needed to be credited.7) On 6/30/15 [redacted] called me and wanted to schedule an appointment for me tofix my teeth! I told him that was not going to happen and asked him what is hegoing to do about the fraudrelant claim to my insurance company and smilegeneration? He said he talked to insurance company and he will be sending acorrected claim to them. But he wanted me to come in so the doctor could megive another option. I said I am not coming in and what about [redacted]and he said he would have to talk to the billing manager to issue a credit. I askedhim to type up and fax me over a letter stating that they sent a corrected claim andissued a full credit to [redacted]. NO ONE HAS EVER CALLED MEAGAIN.8) 7/21115 I faxed them over attention [redacted] requesting the letter I asked for above.9) 7/24/15 I faxed them over 2nd attention [redacted] requesting the letter I asked forabove.1 0) On 7/27/15 I received a faxed (did not state what person sent it) from So.Oceanside Dental "Settlement and General Release"What's wrong with this Settlement"?a) They billed [redacted] $6,000 that they are wanting me to pay and they aregoing to refund me $3,150.00 meaning they are charging me $2,850.00 for workthey did? Which should never have been done in the 1st place!b) Item number 3 in the "Settlement" states to not make negative or derogatorystatements including internet/social activities. I have already made negativereviews on every internet review I could find.c) Item number 4 not file any actions against So. Oceanside dental or the dentist.Which I have already hired a lawyer.If I signed this I would owe $2800 to them and they could sue for all the internetreviews I have made and I could not take them to court or submit anymore complaintsto the dental board.End result is the only thing they corrected and it was by force by my insurancecompany is the corrected claim.The need to credit [redacted] of $6,000 and not charge me for any servicesthey think they did. They ruined my teeth and I have to pay to get it fixed when I onlywent in their to have them put back 1 cap the fell off.Thank you for all your doing for me I am so greatful.[redacted]760 [redacted]
