*** *** Attached is my Benefits GuideBook indicating that fitting, lenses, evaluation, and follow up care 100%. As per them not being a provider for my Davis Vision insurance, if things recently changed, then as several year repeated customer, we c=should have been advised that they were no longer providers for Davis Vision. Something that was never disclosed to any of my family members or myself. I do want for the prescription be released to me and full refund be reimbursed , as the service is covered a 100%. Thank you

Attached is my Benefits GuideBook indicating that fitting, lenses, evaluation, and follow up care 100%. Once again we are an in network provider that has a contract with her insurance company. We are required to collect all copayments and fitting fees as the contract states by the insurance company. We are in compliance with her insurance company. Contacts lenses are a medical device and are not made in any doctor's office. Contact lenses are manufactured by medical corporations and must follow FDA guidelines. Once again I reiterate, the patients insurance was verified before the exam was performed and we have her signed signature on our registration form that she is responsible for all copayments and any charges not covered by her insurance company. A contact lens fitting fee is not covered by the insurance plan, therefore we are required to charge for the fitting fee. If you need any additional information please contact me.
