I was charged for services that were unnecessary and not completed correctly. I was given an ineffective treatment for my problem.
I was charged $800 for a cortisone shot in my shoulder. The shot was ineffective and the *** made mistakes on my bill. I was billed for treatments that he did not perform. I was not told in advance that this treatment might be ineffective, yet cost so much. I had asked to be informed about this in advance, however I was not. After calling and speaking to the billing department, I was told that I could disregard the bill while they investigated my claims, however they immediately sent my bill to collections. I was never offered an opportunity to argue my case. My shoulder was not treated properly and the cortisone shot was ineffective. I feel it is unfair to not have discussed possible charges with me in advance and to warn me that the shot may not work. I feel that this doctor should have dealt with me over the phone and in a more personable matter rather than avoid my calls for help. I would like to have this debt forgiven and this mark on my credit removed. It has and continues to gain interest, yet I will not pay this amount for a service that was ineffective and wrongfully given. I would like to take this to the Medical Board if we cannot resolve this here.

Desired Outcome

Remove my negative credit rating and forgive this bill as it was unfair. The treatment provided was ineffective and wrongfully prescribed.

The dollar amount for the services in question, occurred December 2013 - January 2014. We have documentation of many attempts to work with the patient to set up a payment plan. In April 2015 the patient's account was sent to collections due to non-payment.

The Revdex.com website indicates services should take place within 12 months of the complaint. Therefore, we are requesting, because the services were rendered over 6 years ago and the patient account was sent to collections almost 5 years ago, that this complaint be dismissed based on the amount of time that has passed. Please understand, due to HIPAA Regulations, we are unable to provide supporting documentation.

I would like to have this bill looked at more carefully by a professional to understand and explain that I was billed for procedures that were not explained to me in advance of what was being done. It was agreed that I would be forewarned if there were to be any charges other than my copay. Since I was not told that there would be additional charges, I was under the assumption that the procedures were being covered by my insurance. *** padded his bill, adding absurd and obscure charges, such as an ultrasound and needle placement xrays -- all for a simple cortisone shot that was ineffective twice. *** and SCOI failed to communicate and refused to discuss the bill with me over the phone. I was shocked to receive an $800 bill for cortisone shots that did nothing for me. The proper treatment was a heating pad and ice, not a shot. I hereby deny and appeal this response that is false and deceptive. I am happy to give clearance for the Revdex.com to look at all documentation of this false claim that SCOI and *** are making. It is upsetting that this doctor is able to hurt someone's credit with unfair and unethical billing practices. Please continue to investigate this further until this company agrees to be reasonable and remove this negative report from my credit. Thank you so much.

I would like to register a complaint against Southern California Orthopedic Institute (SCOI). They continue to bill me for doctor visit on 6/18/18 that I am not responsible for. I have had numerous conversations with SCOI and the insurance company, United Health Care Oxford (Oxford). I even set up a conference call on 1/28/19 between SCOI, myself and Oxford. The Oxford representative *** informed the SCOI representative *** that I was not responsible for this billing and that SCOI had used an incorrect code when submitting their claim. That was to no avail as I have kept getting billing from SCOI for the same service. I provided a written complaint to Oxford on 12/26/18, again on 12/3/18 and on 11/20/18. Oxford requested copies of the billing each time. In numerous conversations with Oxford they continue to advise me not to pay SCOI as this doctor visit is covered by the insurance. Each time they have told me I do owe a balance of $22.13.
The most recent phone conversations with Oxford took place of 1/24/20 when I spoke with *** and on 2/3/20 when I spoke with ***. Both told me the same thing, "Do not pay the invoice as it is covered". Neither can understand why SCOI continues to invoice me. They said they would accelerate their effort to get this resolved and would refer this to their in-house legal. Oxford also sent SCOI a letter dated 12/11/18 (attached) wherein Oxford advised SCOI that they were to stop billing me. Part of the letter reads "We have informed the member that he or she is not responsible for these charges". This was also to no avail. SCOI is either inept or corrupt. Either is unacceptable.

Desired Outcome

I am requesting the Revdex.com investigate this matter to see if they can resolve without my having to refer to my attorney. Please advise if there is any further correspondence you require me to produce. Thank you.

HIPAA Privacy Laws limit what information I can relay on this site however I can say that as the Director overseeing the department, proper steps have been taken internally to ensure that the patient balance has been adjusted down to -0- due to miscommunications between SCOI and the Insurance Company. We are also reviewing and addressing why this error was allowed to continue for as long as it did. We apologize for any inconvenience and will reach out to the patient to advise they now have a -0- balance.

I accept their response providing they follow through. Based on the way they've handled this to-date I am not overly confident I will not receive another bill. We will see. Thank you Revdex.com for following this through.

Terrible customer service I was told by(Jocelyn) there was no opportune time to call kept putting me on hold every 5 secs didn't give me any solutions to solve my problems

We were billed $450 for a simple office visitths is excessive since the doctor really did not do anything. We were never told the cost beforehand.

We are unable to locate a patient by this name and address in our system. There is no account number or DOB given. We would prefer a phone call so we can obtain further info into this matter.

SOUTHERN CALIFORNIA ORTHOPEDIC INSTITUTE IS A MEDI-CAL PROVIDER, WHO BILLED AND CONTINUES TO BILL A MEDI-CAL BENEFICIARY. THEY NEVER BILLED MEDI-CAL.
On January 12, 2016, I visited Southern California Orthopedic Institute for an office visit with one of their surgeons. I provided them with my primary insurance card, and for my secondary insurance which was Medi-Cal, I provided them with my Social Security number. I had not yet received my Medi-Cal card, but was in their system. They were able to confirm that I had both insurances, and that I owed nothing upon checkout.

In April 2016, Southern California Orthopedic Institute sent me a bill for $566. Please note that Southern California Orthopedic Institute is a Medi-Cal provider, and I am a Medi-Cal beneficiary. It is ILLEGAL to bill a Medi-Cal beneficiary.

On May 2, 2016, I sent a certified letter to the provider warning him that if he continues to bill me again, he will get in big trouble. His office received my certified letter and signed the signature card on May 4, 2016. Instead of billing Medi-Cal the difference of what was owed after my primary insurance didn't pay, they had the audacity to play with fire. They continued to bill me for another year and a half.

I have complained to Medi-Cal and they are still investigating Southern California Orthopedic Institute. Any day now, Dr. Jaivin and Southern California Orthopedic Institute will get arrested. I don't worry about that part. The issue is that Southern California Orthopedic Institute had the audacity to send the debt to AWA Collections with an interest of $73.79, and threatened to ruin my credit. I received a bill just this month.

Southern California Orthopedic Institute needs a wake up call. This is not OK. This is not my debt. They should have billed Medi-Cal, because that was their job. Not mine.

Attached, please find the Medi-Cal codes they have violated.

Desired Outcome

Stop sending me bills. Stop threatening to ruin my credit. IT IS ILLEGAL TO BILL A MEDI-CAL PATIENT, ESPECIALLY WHEN MEDI-CAL AND I HAVE CONFIRMED FOR SURE THAT YOU ARE A MEDI-CAL PROVIDER. Also, if any negative remarks have been placed on my credit report, you must clear it up before *** Southern California Orthopedic Institute get arrested. This is your last chance. Finally, I need a zero balance statement from you along with an apology for sending bills to a Medi-Cal beneficiary. Please note, all forms of communication received through this bureau will and can be used against you in a court of law, and will most definitely be forwarded to Medi-Cal to decide the proper criminal violations to be charged against you.

The patient's claim that she provided us her secondary insurance information could not be substantiated. To date, the information has not been provided to us. It is past timely filing statutes for the claim, so we have contacted our outside collections agency and rescinded the account. We have also closed the account in our books, and advised the patient that she does not owe any balance on her account for this date of service.

The business is lying. They knew from day one that I had straight medi-cal as my secondary. As soon as I received their erroneous bill in April 2016, I called and provided them with my medi-cal card number and issue date. What more is *** claiming he needed to substantiate my eligibility? Furthermore, I was the one who contacted the collections agency and notified them about their fraudulent attempts to collect money from a Medi-Cal provider. I also provided all my documents to the credit agency, which showed I was a Medi-cal beneficiary. It wasn't because *** contacted them that they rescinded the account. It was because I contacted the debt collector and provided them with everything. Assuming *** didn't know as late as April 2018, the collections agency must have already shown him my proof of eligibility by now. So, ***, if you're going to apologize for illegally billing a medi-cal beneficiary for almost two years even though you received my certified letter in April 2016, then at least do it in a classy way. Don't make up facts that you couldn't "substantiate" it. Attached, please find my medi-cal eligibility letter, again, which I requested from my county right after I received your collection agency's erroneous bill.
