On September 15, 2015, Mr*** accepted and signed a treatment plan and payment agreement
with the Dental School making a $down payment toward treatment
Charges of $were incurred on August 25, and the Dental School filed a claim on Mr***'s behalfPer SDM policy we cannot refund an amount unless we have received payments in excess of chargesMr***'s insurance company is Delta Dental and does not usually assign benefits but rather pays directly to their participantAlso, his insurance policy is through the State of Illinois which has not paid claims with dates of service after February Since no other reimbursement has been received on Mr***'s behalf, his refund is $($less $90)
After receiving a credit statement dated 2/2/2016, Mr*** signed and requested a refund be madeWe received this request in the Clinic Business Office via US Mail on 2/12/We processed the request and in accordance with University procedures sent it to the University's Accounts Payable department where a check will be disbursed
If at a future date we receive a payment based on Mr***'s 8/25/claim, we will immediately refund the amount reimbursed to Mr***
Copies of the documents mentioned above can be forwarded if Mr*** provides written permission

I would still like to know why Dr*** did nothing this whole time except take my money and never returned my callsAlmost six months has gone by, I could of almost had implants completed by now if I would of went to a REAL DENTIST!!
I am asking for $for time, travel, and interest on my money
I want everyone to know this is a SORRY place to get dental care, it appears that the main thing they are taught is to explain the procedure and take you're money
