This is in response to your letter of which I received via email today August 16, as you have an incorrect mailing address for usI have had a meeting with this resident and I am waiting for documentation from the resident for her pets She was not the only individual who received a violation and is not being singled outThis complaint is for two different issues and I am not able to address both in this format due to confidentialityAll residents are required to have up to date vaccinations for any animal whether the animal is an assistance animal or notSincerely,

I could argue facts and dates but I no longer wish to engage with this companyI have talked to other health care professionals who tell me this was not done in a professional or satisfactory mannerI did receive some of the paper work today by UPS, (which my wife and I obtained from the various Doctors and am giving this paperwork to the new prosthetic company tomorrow)This process does not take days to complete especially when the insurance company is telling us to submit the paperwork to get approval

prescription till 9/9/from your PHYSICIAN ASSISTANE. The ordered prescription must be signed by your primary care physician Dr *** We did not received your medical records till 8/25/ Dr*** and 9/10/from Dr*** and Dr***.
