I have forwarded my letter as response from Southwest Family Dental letter.With all due respect Revdex.com, how I see it, it is “my words against their words”I am not sure on how this kind of process will help me, but I draw your attention with high hopes that your office will help people like me who is willing to voice out the concerns against a business, that is sending billing “just like” thatI am one of the consumers that sometimes does not pay attention with the bills we received as life is so fast in a daily basis and we get so busy, but this time I decided to pause and stopI am not willing and it will hurt me if I don’t let this people know that it is not easy to give your hard earned money just because they let you out of the loop during the process.I am sincerely asking for your help to resolve this.Thank you so much.Regards, [redacted]

On December 14, 2016, we had the pleasure to treat Mrs***Patient was present for Comprehensive exam, x-rays taken and prophylaxis with fluorideFuture treatment was discussed with herPatient scheduled an appointment for December 19, Operative treatment was performed for teeth #2, and When patient check out we requested the payment of $deductible and patient refused to payMrs*** requested another appointment to continue treatment and she was given the date of December 29, Mrs*** did not showed to the appointmentWe have a policy if a patient does not call to cancel appointment they need to pay a fee of $Mrs*** signed the policy as wellAlso, a Pre- Authorization was mailed for scaling and root planning to Guardian Insurance. On January 3, Mrs*** called to schedule an appointment as a limited exam because she was still sore from the fillings on the upper rightAn appointment was given to her for the next day, She came and was treatedAgain, we requested the $deductible and she refused to payWe mentioned to the patient for the next appointment she must pay the deductible at the time of the visitMrs*** came on January 31, and she gave us the payment for the deductible but she refused to pay for the balance in the account of $We received the explanation of benefits of the insurance stating patient need to pay $Mrs*** could not believe insurance was charging the amount of $when they were supposed to pay 100%We told her to call the insurance and find out We always go by the explanation of benefitsMrs*** stated that she won't pay the difference for the treatment performed.Sincerely,Wen L***Dentist
