Was seen by Dr. [redacted] on 1/25/2015 for pelvic and back pain and urinary problems. Gave a urine sample for analysis. Dr. [redacted] without any examination said that she would issue me an "expeditied" referral for urology v.s. a regular referral and said that she was going go give me a precription for cipro for 14-28 days to treat possible Prostatitis. When the discharge nurse arrived with the presciption it was only for 10 days of cipro. When I stated that the Dr. had stated that she was going to issue a dose for 14-28 days, the nurse state that "we don't give cipro for longer than 10 days". I stated that it was the Dr.'s directions and not something I decided. The nurse left and came back with a corrected prescription for cipro for 14 days. I consider this a medication error that would have been made if I had not questiond it and referred the nurse back to the Dr. I was also told by the Dr. and written in her own handwriting on my discharge orders that she was giving me an "expeditied" urology referral and would have an appointment within 10 days and to call the urology office to make/verify the appointment. I called 2 days later to give sufficient time and told by the urology office that only a "regular" referral was submitted and couldn't be scheduled for at least 2 weeks and maybe later. I explained the "expedited" referral promise and that I had it in writing and they told me I would have to discuss it with Dr. [redacted] office. I called Dr. [redacted] office and spoke with a person named [redacted] who first said that she sent a "regular" referral only and when I told her I had a written statement by the Dr. for an "expedited" referral she said I was mistaken. She was confrontational and unhelpful so I asked to speak to her supervisor and she said that I couldn't that her supervisor was busy, she then said that she would transfer me to the supervisor and while waiting the call was disconnected. She later left a voice mail stating that she had transfered me to the wrong number and that's why I had been disconnected and made no further attempts to arrange contact with her supervisor. After filing a complaint with [redacted] of [redacted] I was contacted by SMA and told that a new expedited referral had been issued and to call the urologist and make an appointment. I made the call the the urology office said they could not make an appointment at that time and that someone should contact me at some point (non-specific). The person at SMA had told me that if there were any problems making the appointment to call her back but conveniently left no contact number. When I called SMA, the new person I spoke to said that the referring Dr. and the referral Dr. had to consult before the referral could be approved and an appointment scheduled...all new information. She said to call back in 2-3 days but not who to call...SMA or the urology office and when I questioned her about that she said she didn't know and I could call either. Now 3 days into what was supposed to be 10 day process at most and no indication on when it would be resolved and who would let me know. I called SMA again to get some clarification and some direction as to how to proceed and was never given and direct answer. So my complaint is the following:
1.I was seen by a doctor who never performed any kind of physical examination before issuing me a referral for urology and a prescription which was not the correct one that she stated was required and only caught by my questioning.
2.The discharge nurse was unprofessional in handling my concern for an incorrectly issued prescription.
3.The promised "expedited" referral was incorrectly entered as a regular referral and complicated and slowed down the referral process.
4. That [redacted] the office representative made inaccurate and contradictory statements regarding my case with unprofessionalism.
5.That no one at SMA could agree on the process for my referral and how it would be corrected and how I would be notified.
