I am responding to your letter dated March 9, 2016 regarding the above-mentioned complaint. These are the steps that we, as the Spectrum Medical Group, Southern Radiology division, billing office previous took in an attempt to resolve the patient’s medical claim:We attempted to bill insurance...

with information provided by ordering physician, and well within insurance filing limit requirements.  The claim was denied because the patient information we were provided does not match what the insurance company has on their system.  NOTE that the patient has no direct contact with the provider of services at the time of service, therefore, we are at the mercy of the ordering physician to provide us with accurate patient billing information.We sent statements to the patient on 11-5-15, 12-12-15 and 1-19-16, requesting correct billing information.  The patient neglected to contact us to resolve the issue until the date he filed complaint with you and AFTER he was contacted by the Collection Agency to whom his account was referred.  NOTE that the facility where the Radiology services were rendered has also sent repeated requests for correct billing information, and they have been unsuccessful as well.   Although all entities attempt to share the information as much as possible to alleviate the burden on their mutual patients, it is the complainant’s responsibility to provide accurate information to all providers so that they may file insurance claims on his behalf.   We have since coordinated with the facility where services were rendered and jointly contacted the office of the physician who ordered the radiology exam to obtain the corrected information needed. I am not at liberty to create a Revdex.com on-line account on behalf of my client and can only present the facts as it pertains to the billing and collection of their claims.      If there is anything else we need to do to resolve this complaint, please let me know.  Thank you! [redacted] Operations
