Initial Business Response /* (1000, 7, 2015/07/29) */
First let me apologize, I probably trashed the first notice, thing it was advertising or junk mail. In 37 years in practice I have had no contact with the Revdex.com.
Secondly, let me try to explain and set the record straight. The patient's...

mother had been a patient in our since 1997, and we have treated other members for several years. we had always treated her and have a very good track record of helping her with alot with previous problems wit her other dentist. We have always worked with he to explain treatment, cost, and expectations for payments. Treatment of her son from 3/14 to 12/14 was no different from the above. The problem as we see it is miscommunication between the father who brought the patient in for the treatment and the mother who controls the finances. The mother filled out all the consent and paperwork for treatment 1/3/13 for and did not show for the appointment 1/10/13. More than a year later, the father brought the patient in for a routine cleaning and exam and again on 11/4/14 when xrays were taken and a treatment plan was developed.
treatment was explained to the father consisting of 5 fillings, 2 of which had the possibility of complication requiring additional procedures ie pulp treatments because of the severity of the decay. The father broight the child in for treatment and was explained any changes at the time of the complication on 11/19, 12/09/ 12/11 and 12/17. Treatment was quite successful with few complications because it was completed on a timely basis!! in fact the father was near or in the room at the time of treatment!! Never at any time was there any objection to any treatment rendered. All treatment was completed in a professional, timely, and safe manner. If at any time any resistance to treatment had been brought up, we surely would have responded.
When payment was requested at eash appointment, the father explained that "my wife takes care of that" Since she had always done in the past, all insurance was billed($1732) and all insurance payment ($1041) was received. The mother made a single payment of $100 at the end of treatment. She was in the same network and insurance as she had been it the past,, so we simply asked for arrangements for the $616 balance.
Only after her disatifaction wit her companies payment did she begin to be upset with the treatment rendered. We are sorry she did not receive the info from her husband, we fully disclosed all expectations. It is not unusual for a patient to have to pay $616 for over $1700 in treatment in today's insurance environment. Remember she payed NO co-payment at the time of service. After 90 day and with all her hassles about the account, in was turned over to the attorney for collection. we had made every attempt to rectify the account, but was met with hostility, and badgering about "consent forms", after they willing brought the patient in for treatment and full disclosure of all treatment possibilities.
Your help in this situation is greatly appreciated Thank you
