Matter has been resolved. Please see the attached email and letters that were to provided from Sports & Orthopaedic Specialists and [redacted] back on 7/7/15. Not sure why party continues to dispute claim. We are more than welcome to assist with anything else but party would need to provide documentation supporting their claim. We are not showing anything involving [redacted] or [redacted].

Customer issue has been resolved. Individual called in regarding the matter on 6/30/15. Matter was resolved within a day.

I have reviewed the response made by the business in reference to complaint ID [redacted], and have determined that this proposed...

action would not resolve my complaint.  For your reference, details of the offer I reviewed appear below.[Repeated calls were made to Dr. [redacted] office between May 15 and June 29.  Since filing this dispute, SOS/Dr. [redacted] office has been cooperative via email.  However, there are still outstanding/unresolved issues at this time which need to be corrected.  Unresolved issues are as follows:1.  I have requested a letter explaining the clerical errors be sent to my home mailing address; this has not been received.  2. Due to Dr. [redacted] office clerical errors, there is an outstanding bill due --- in my name, address, and date of birth --- from [redacted] which has still not been completely resolved.  It is still on "[redacted]" online billing system and needs to be deleted.  3. Due to Dr. [redacted] office clerical errors, [redacted] cases are open in my name and address .  This needs to be cleared and deleted with confirmation by [redacted]. 4. Due to Dr. [redacted] office clerical errors, [redacted] has been trying to schedule appointments with me erroneously.  I would like to have written confirmation mailed to my home address (on file) that this gets corrected.  I have already provided aforementioned information to [redacted].  Note - [redacted] has stopped calling me to pick up a prescription called in by SOS.  [redacted] has also stopped calling me regarding erroneous bloodwork affiliated with SOS." ]

Review: Over the last couple of months (since May 2015), I have been contacted by phone and mail from multiple medically-related organizations having to do with erroneous treatment, billing, insurance, etc. of which I am not

responsible for nor do I have knowledge of. In all cases these have been tied to "SOS" (Sports and Orthopaedic Medical Specialists).

There is apparently a mix-up with another individual's or individuals' insurance information and/or billing responsibility.

My personal information is erroneously being shared outside of SOS's office and now I am facing collection activity for bills of which I am not and refuse to be held accountable for. Calls to SOS's office and billing staff (working for Dr. [redacted]) have not resolved this problem.Desired Settlement: As I am now facing collection activity, I am seeking to have this issue resolved immediately by Dr. [redacted]/Sports and Orthopaedic Medical Specialists with a written explanation of what has transpired, corrective action(s) taken with written confirmation of same for my records.
