The doctor's office is: [redacted], Kirkwood, MO 63122, [redacted]. I'd like them to correct my medical records, clearly there is inaccurate information in my file. Their mistakes about whether or not I have prescriptions, missed appointments, etc. I'm scheduled for surgery and had a medical professional ask me to verify my address. They had some address around Stover, MO, a town I've never lived in or near. I want notification and verification that this has happened.

The patient and provider did not agree as to how often the patient needed to be seen in the office for follow up care.  When this was addressed with the patient, he stated that he could not afford to come in unless he had a problem and he would be finding a provider closer to his home.  At that point, it was decided that it would be best to terminate the physician/patient relationship.

Luke’s system. I do like my new doctor. I was very concerned that SSM Heath
Care was getting me mixed up with another patient that has a similar last name
as mine. The funny thing about this whole deal was that SMM Heath Care did not
seem to care about my concerns so that is why I moved on to a new heath care
system and a new doctor. At this point I am happy with my new doctor and his health care system
and there is nothing you can do for me. I realized that you are going to close
out this complaint and that I will get a letter from the Revdex.com in about two
weeks.I hope by reporting it to the Revdex.com, that no one else has the experience that I had with SSM Health Care.
