The two complaint issues were reviewed with findings as follows.St. Clair’s Human Resource (HR) department completed a thorough review of Ms. [redacted]’s claim of possible identity theft due to an old email address that she never provided to St. Clair Hospital. Besides the September 4, 2015 date that Ms. [redacted] applied for a position at SCH, she also applied on two additional occasions. Once on March 24, 2015 utilizing the [redacted] and once in May 2009 utilizing an email address of [redacted]. Ms. [redacted] may have forgotten she submitted an application in 2009. Her name and social security number matched all three applications, but the email address was changed for the 2015 applications.In response to the second part of the complaint, St. Clair Hospital never sent Ms. [redacted] a statement for any services to date. Ms. [redacted] was admitted to St. Clair Hospital on September 29, 2015 and discharged on October 9, 2015 (account number [redacted]. The services rendered were never billed to the patient. Ms. [redacted] applied for and was determined eligible for Medical Assistance effective September 29, 2015. The hospital is in the process of billing Medical Assistance for payment. Our customer service department has no record of the patient calling to discuss her account or any disputed balance.Base on the findings above, St. Clair’s position is that Ms. [redacted] did provide St. Clair Hospital her old email address in the past with her May 2009 application and that St. Clair Hospital has never sent Ms. [redacted] a statement for any patient responsibility. Verizon wireless has a strict 14 day return / exchange policy on all device sales. We provided full support to the customer in regards to her issue with her phone. We did in house tech support and helped customer with Verizon tech support over the phone. Customer still claiming to have issues...

with battery. We are unable to process a return outside of 14 days. Verizon provides a 1 year warranty on all devices, her phone is covered with in this warranty. Customer can receive a replacement phone through warranty at no charge. Customer has refused this solution.

Review: I applied for a job at St. Clair Hospital the date of 9-4-15. While applying, I noticed that my old email address was in their computer system. This email address is not used anymore! I never been at this hospital before, only to apply on the date mentioned above. Someone could be using my identity. The email address in question is [redacted]. Shortly after I was admitted 9-29-15 to this hospital( St. Clair Hospital) with no reason or paper work given. After discharge 10-9-15, I received a bill in the mail with false statement claims or services. I called the billing department to clear up the matter, I received two more bills with services never given.Desired Settlement: Remove false claims from credit file and never be contacted by the hospital again.
