Dear Sir or Madam, Re: Complaint by *** ***Mrs*** came to my office to establish care on 09/21/and had an annual physical at that time (which her insurance paid for)As the term infers, an annual physical is paid for YEARLY and would only be available to her again in September
2017-it also implies a general cheWITHOUT ANY TYPE OF MEDICAL INTERVENTION OR MEDICATION WHATSOEVERThe date of service to which she refers (March 2, 2017) per our records demonstrates that she had a urine HCG (human chorionic gonadotropin sent to the CPL (Clinical Pathology Labs) due to urinary tract infection that had been ongoing for the previous three weeksWe also sent her for an ultrasound of her kidneys at that time and referred her to a specialistOn May 3, the patient was seen for multiple issues in office including: autoimmune thyroiditis, insulin resistance, obesity, kidney stones, anxiety, hyperlipidemia, pain in right leg, pain in left leg and a sleep disorderThis is not part of an annual physical-nor did she have any bloodwork drawn in our office at the time of visit but requested her lab orders to be sent to CPL PRIOR to her visit to avoid the trip to my officeAll labs ordered at that time were done so specifically to address the above issues and nothing more. We have coded her visits correctly, referred her to our billing department and finally recommended she speak with her insuranceUnfortunately, we have no control over what insurance policy our patients chooseHowever, we do try to educate them from the get go that their insurance may not cover everything and they will have an out of pocket expenseThis is not the doctor's responsibility and we will not be held liable

I am rejecting this response because:Despite not having my authorization, *** *** and her StLazarus Family Practice released my personal medical information without my consent to the Revdex.com in their first messageI never signed a HIPPA release form and they never requested my permission to release my personal medical information before sharing my informationNot until after they already released my private medical information they then sought my authorizationI do not want to sign a HIPAA after the fact and do not want to in any way validate what they did
