Thank you Ram for the dependable and professional service. I called Ram to do a furnace and duct cleaning job because we just finished our renovations. He arrived on time, and very careful in handling his equipment as well to prevent scratching our walls with the hose. He gave me lots of information as well so we understand mire about our furnace and hot water tank condition.
I have a one year old and I felt very confident in Ram's work to move back in to our home as thr air is now cleaner.

My dad also hired Ram after to do cleaning for his own house and 2 rental properties.

I had a question about our water tank and I felt no hesitation to call him as I know he would be very helpful like a food friend. Because many people in this field can be rude and refuse to answer questions until you pay them to come to the site.

Thanks again Ram and I will continue to use your services in the future.

Review: I have insurance with [redacted] and I pay $664 dollars a month. I went to the [redacted] center for pain management in the St. Luke's Roosevelt hospital center and went to [redacted]. He recommend I get a [redacted] in my back. So I made the appointment and got the ok from the insurance company but when I asked the Dr's [redacted] would it be all covered ? she told me it would not cover it all. Then I asked her how much would it cost and she told me she did not know and call the billing dept. I called them and they could not tell me also. I think it is totally unfair that they are aloud to be able to do this. So I have to have a [redacted] in my back to relief my pain in my back and I don't know how much this is going to cost me on top of paying for health insurance of $664 a month. Could you please help me and find out how much it will cost me or if the insurance will cover it all. For $664 dollars a month I would have to be dying in a bed before it would be covered.Desired Settlement: That my Insurance would cover the cost of the [redacted] or tell me how much the is going to cost on top of my insurance. Review: On 5/**/13 I had a consultation with [redacted] about a specific health concern. Before the consultation, we discussed the fact that I did not have health insurance and requested how much the consultation would cost without health insurance. I was informed that the consultation would cost $100. I paid for this consultation and have a receipt to prove it. After the consultation, [redacted] informed me that he would like to perform a procedure on a later date. He requested for me to get a blood and urine analysis at "Continuum Services Patient Service Center." I paid for the service in full of 105.00 with a receipt to prove it. This blood and urine analysis was forwarded to [redacted].

I received two bills in the mail for services performed on 5/**/13. The first bill is of the amount of $225.00 for "Cyto Fluids Wash" service. I did not receive that type of service on 5/**/13 as that day was strictly a consultation and no other services were provided. The second bill was for services on 5/**/13 was for a total of $145.00 for the following services: "Venipuncture," "Cult Bact Qnt Clny Cnt Urine," "Urinalysis Auto w/micro." As was stated before, I had no other services or informed of any other services provided on 5/**/13. 5/**/13 was limited to only a consultation with a request to get a urine and blood analysis completely at a separate company (Continuum Services Patient Service Center). Without insurance, I specifically spoke with my doctor that I would need to know the exact billing and amount that would be charge for the consultation. I was never informed of any other service that I would be required to pay for. In addition, the second bill for "Venipuncture and Cult Bact Qnt Ciny Cnt Urine" is exactly what Continuum Services Patient service Center was testing for. Why would it need to be done twice and why would I be billed for it twice by two different companies looking to accomplish the same analysis? I'll pay for one, but not the other that was not agree to during the billing agreement between me and [redacted].

I had a procedure to be done on 6/**/2013. The [redacted] and I discussed the total amount to be charged for the procedure as this procedure can be extremely costly and without insurance I requested full disclosure of the amount to be billed for the procedure. After the Doctor received the urine and blood analysis from Continuum Services Patient Service Center, he could continue with the procedure. I paid the ** in full the total amount agreed upon and due at the end of the procedure of $200 for his services. After paying, the [redacted] told me that I was completely up-to-date on billing and reviewed the results of the procedure. I then received in the mail two bills from St Luke's Roosevelt Hospital for the amounts of $175.00 and $225.00 for services rendered on 06/**/13.

The bill for $175.00 was for the following services on 06/**/13: "Venipuncture," "Cult Bact Qnt Clny Cnt Urine," "urinealysis auto w/micro" and "Basic Metabolic Panel." I was never informed of these services and I never agreed to these services. Additionally, these services are not needed as they were performed by "Continuum Services Patient Service Center" on an earlier date. Another aspect of this is that these services would be needed to be done at an earlier date as the analysis is done in a laboratory and takes time to complete. The analysis would be needed in order for the ** to do the procedure so why would it be done on that date, processed and the information available post procedure? And the analysis was done days before by Continuum Services...

The other bill for $225.00 for services on 06/**/2013 was for "Cyto Fluids Wash." This is another unknown service that I did not agree to and I was not informed of. Before the procedure, the Doctor informed me that the total cost of the procedure would be for $200.00 and was paid on 06/**/14, the same day that I had my procedure. I paid in full the $200 for the procedure I had. I have records and receipts

I also want to make it clear that I was billed for services on 06/**/14 while my procedure was on 06/**/13. Why am I being billed for two services performed on a date that I wasn't even present for?

I tried to get this resolved with St. Lukes and they said that I owe this money. They didn't give me any other explanation other than these services have been rendered and I need to pay it. They have forwarded my bills on to a collection agency. The collection agency is "NCO Financial Systems, Inc." I cannot resolve this issue with them.

In conclusion, I am being billed for services that I was not informed of, never received, was unnecessary and was claimed to have received on a date that I was not present for and separate from the date of the procedure. My doctor fully disclosed all billing items and was agreed upon before services rendered. Any additional services were not agreed to and I was not informed of.Desired Settlement: To not be charged for services that were not agreed upon, not rendered and not applicable to the services requested and adjustment to my credit history

Review: On 12[redacted]/12 $158.31 co-payment was submitted twice by mistake. I saw a double charge on my bank account I called the customer care line at [redacted] to find out if a refund can be issued for a second charge. I was ensured that I will get a refund in two weeks. On 02[redacted]/2013 I called again and again I was told the system is showing a refund due and refund check will be mailed out in two weeks. I haven’t received anything and I called again on 06/**/2013. I spoke to [redacted] at [redacted] who told me that this time the refund will be mailed out shortly. Then on 08/[redacted]2013 I called [redacted] again and this time she told me that the refund will be mailed next business day. I called again on 09/**/2013 and this time I was told they don’t know when the refund will be mailed out. It is obvious that this company made a mistake by processing two the same charges and not refunding the $158.31 to me. I have made numerous attempts to get my refund, but they refuse to do so.Desired Settlement: I’m extremely disappointed in this hospital’s billing department and asking for the refund, $158.31, to be processed ASAP!
