This letter is in response to your letter dated June 3, 2016 that we received regarding thecomplaint issued by [redacted], in which she stated she received collection notices for abalance she had with us.In researching this issue, I found the following:• Ms. [redacted]' son received services with...
us on July 22,2014. This visit was billed to herinsurance and we received payment from BCBS on October 23,2014.• BCBS left a copay of$20.00 as patient responsibility. This balance was moved to ourstatement vendor on December 3, 2014.• Per our statement vendor, statement notices were issued and sent to Ms. [redacted] onJanuary 26,2015, February26, 2015, March 30,2015, May 1, 2015, and May 15,2015.• Because no payment was received from Ms. [redacted] for the balance, it was moved to ourcollection agency on April29, 2016.• We have called Ms. [redacted] to review these details and are working with her directly forpayment and resolution to this balance.If you have further questions regarding this matter, please call me.
