Attached are the acknowledgement, consent and invoice MrsL*** signed prior to receiving her treatmentsIn it she acknowledges that we cannot give refunds, that additional treatments may be necessary, there is no guarantee of any treatment and results may vary Please review

I have had Botox treatments times in the past yearsIt has always worked to paralyze the muscles fully and has lasted three to four monthsI have gotten a wrinkle free forehead every single time, lasting to months, with only units of BotoxI have two close friends who get Botox treatments due to frequent migraines that have not responded to any other medicationThey get full paralysis of all muscles treated and their migraines fully abateI have a co-worker who has benign tremor that effects her entire bodyWhen she comes to work after treatment she has absolutely no tremorand 1/months later tremors begin to re-emerge and she then goes for another Botox treatment once again she is tremor freeBotox works.I am certain that what I was treated with at AZ Laser was primarily water because I look as if I have had no treatment. I have read many negative reviews on *** about AZ Laser on ***In one review the reviewer states that an employee of AZ Laser went to another room to fill syringes for her treatments, the reviewer/customer went to look for the employee and when she opened the door she saw the employee at a dirty, rusty, sink filling the syringes with tap water. I am demanding a full refund of my $dollars paid on 04/08/2016. This company is willfully fraudulent

Prior to treatment Ms*** was advised in writing, attached is the signed consent form, that there are possiblities of blistering, scarring, hypopigmentation(lightening of skin), hyperpigmentation(darkening of skin) during this treatment Also, Ms *** acknowledged in writing that AZ
Laser Clinic cannot provide refunds and that any credits could be applied to other treatments As of this date, Ms*** has a credit of $ We could apply this amount to other treaments or in this case we could refund the amount of $to Ms***

Although I appreciate the willingness of the business to have me used the money I spent, I cannot accept this responseI attempted several calls to this business regarding my experience prior to contacting Revdex.com and my credit card company to try to clarify my experience and have a resolutionMy calls were not followed up properly, but now that a complaint is in place I received a call back from the business trying to have me come in for treatment saying that indeed there was a mistakeThat's the resolution I attempted to get at with all my calls initially to them and after not receiving an appropriate response I was forced to contact my credit card company, the Revdex.com and AZ attorney's General office.Moreover, I do not feel comfortable receiving treatment of a medical nature at a place where my medical chart appears to have been noted incorrectly and no effort was done to follow up with me regarding this or clarify what exactly happened.I do not see a difference in the amount, I would like $refundedI had paid for $for a syringe of Restylane that was not used and I paid an additional $to have this credit transferred to be able to purchase a Voluma which I never used either. Thank you

MrsL*** purchased a package which included a V-Lift, Secret RF Treatment and units of Botox During her 1st appointment with us she was administered units of Botox Units where to be complimentary and units was discounted as part of her package price At this point
she has opted not to continue with treatments and stay with the units of Botox already administered. As for the effectiveness of the treatment, all Botox in our office is administered according to manufacture standards MrsL*** acknowledged in writing on a consent form that effectiveness of Botox varies from person to person and additional injections may be needed to achieve a satisfactory result. Our retail price per unit of Botox is $12/unit which totals $ Although we have a no refund policy, which MrsL*** acknowledged in writing, we can refund the difference of the unused portion treatments which comes out to $

For your reference, details of the offer I reviewed appear below.This standard contract for lightening and/or removing veins does not include and acknowledgement of burns so severe that a "hole" is left in my leg (as seen in photos)After the first treatment, I burned so badly that the technician could not treat that area againInstead, she treated the inner leg above my knee which also scarredNo notations were made in my file that this had occurred either timeThe result of this permanent disfiguration is clearly due to negligence of the technicianI had one treatment and this was the result, therefore I should be refunded the full amount***, the receptionist, also agreed that she would not want a "credit for other services either." As she stated, "I don't blame you." In an article in The publication "Latino Persoectives", the Tafoyas were quoted as saying "they cater to Hispanic males and females..."Being Caucasian, perhaps they are unfamiliar with the difference in skin tone, which could very well have led to this disfiguartatiion on my left leg.(article attached).I have tried numerous times to resolve this issue, however *** Tafoya has not responded as of late, and also misrepresented herself as "just an independent contractor", and has nothing to do with this business (which may be the case since she is never ever there). Ironically, she shares the same last name as the owner and clearly had a personal interest. This business has been extremely unprofessional, negligent, and misrepresented themselvesI politely went in the first time to request a refund, waited for another patient to leave before discussing the problem and allowing *** to take photos of the damage, therefore giving them the opportunity to resolve the problemThey feel they are entitled to payment for a service that has left my leg looking worse than before I went inI disagreeI believe they should, in good faith and in good business, refund the $for vein removalI did not ask to be refunded for the Rejuvaderm injection which was another $I certainly feel I should fairly receive the vein removal funds of $however. an accounting of services would be appreciated alsoI received one treatment, burned, returned for a second partial treatment, and apparently there is only $in my account? Again, *** says this has "never happened before" in her email to me (which I can also provide), but according to reviews that I have read after this horrible experience, it has in fact happened to others. Thank you for your attention to this matterI look forward to hearing fromAZ Laser again soon (through Revdex.com) , hopefully with a realistic resolve.
