The COBRA qualifying event information for Ms*** was entered into the COBRA Client portal outside of ***. The entry contained a typographical error in the date of the participant’s termination. The date was entered as and it should have
been 2018. Based on the date being entered into the *** administration system, *** sent a letter to Ms*** asking for prior premium payments dating back to 2017. Ms*** called to ask why and *** contacted the employer for verification of the correct year of termination. The year was corrected to as per the employer’s direction; however, this change in termination year resulted in a change in the premium amount due. *** advised Ms*** via written correspondence that there was a balance due to cover the first month’s premium in full. ***’s procedure is that when the initial premium is paid in full *** will send a request to the insurance carrier asking them to activate the COBRA coverage. When the entire first month premium was received in full, *** contacted the carrier to request activation of the COBRA coverage for Ms***
