.Mr [redacted] came into our office for an eye exam on July 11,At that time he received an eye exam and purchased a pair of progressivelenses and a Nike frameHe was educated on how the progressive works, and itwas his choice to purchase glasses from usWhen he picked up his new glasses,he was asked to put them on, the fit was evaluated and again he was educated onhow the lenses workHe was also told that sometimes the progressive lensestake a little bit to get used to, but don’t hesitate to contact our office orjust come by, that a little adjustment can make a big differenceAs far as ourrecords indicate he never came in or contacted our office.I encountered Mr [redacted] again on Monday, April 27, Hecame in complaining about his visionI explained to him that after monthsthat maybe his prescription had changedHe scheduled an eye exam for laterthat dayHowever, he called back to cancel that appointment, because he wascalled in to workHe then returned on Wednesday, April 29, after going to [redacted] for an eye examI checked the prescription from [redacted] against hisglasses and they were not the same prescriptionI tried to explain to Mr[redacted] that after months that his eyes had changedHe started to become irateto me, demanding how I was going to “fix this,” that I needed to remake them atno chargeI tried to explain that the glasses had a day patientsatisfaction warrantyBut he was becoming more and more irate, threating to goonline and give us a bad review, if I didn’t do as he askedI asked Dr***to step in, at that point things escalated very fast and the police werecalledMr [redacted] left our office yelling obscenities and he threw his glassesat us as he was leaving

I explained to him that after 20 monthsthat maybe his prescription had changed. He scheduled an eye exam for laterthat day. However, he called back to cancel that appointment, because he wascalled in to work. He then returned on Wednesday, April 29, 2015 after going to[redacted] for an eye exam. I checked the prescription from [redacted] against hisglasses and they were not the same prescription. I tried to explain to Mr.[redacted] that after 16 months that his eyes had changed. He started to become irateto me, demanding how I was going to “fix this,” that I needed to remake them atno charge. I tried to explain that the glasses had a 90 day patientsatisfaction warranty. But he was becoming more and more irate, threating to goonline and give us a bad review, if I didn’t do as he asked. I asked Dr. [redacted]to step in, at that point things escalated very fast and the police werecalled. Mr. [redacted] left our office yelling obscenities and he threw his glassesat us as he was leaving.
