I attempted to get an appointment since December“The Doctor will not see you until your balance is paid down.” “The Doctor will not release the appliance.” They only called to provide service AFTER receiving the complaint from Revdex.comLet's recall that this oversight of me not having insurance was an error on my part, yesBut they did not catch when they “as a courtesy” billed insurance two months laterOnce lack of coverage was discovered we agreed to a payment planRegardless of Star Dental not taking responsibility for their inaction to this pointThey did see me on March 25, to attempt to complete serviceThe appliance did not fit and does have to be sent backThis is not my fault and should not be charged (as they indicated)I do have a follappointment in order to complete the workWhich is the main point of the complaint, and if completed as scheduled will resolve the complaintAlthough they are taking action finally to resolve the matterThey have a history of canceling appointments on Friday afternoonsI will not consider this matter resolved until the work is complete Sincerely, [redacted] ***

Response to complaint # ***, we faxed all supporting documents to fax # *** including signed consent form for services stating ptagrees to pay regardless of insurance; signed and dated insurance & financial policy stating that we bill insurance as a courtesy, but that it is
patient responsibility to pay; signed and dated financial payment contract; current copy showing proof of insurance from *** *** of Idaho dated 10/02/2015; patient ledger showing pay dates. *** *** was first seen 08/21/2015, at this time *** presented an insurance card Payment arrangements and consent for services were went over and signed *** wife *** signed a payment agreement dated 09/05/ At this time *** stated that she would pay at least $and wrote in corresponding pay dates (see in faxed material).*** stated that he "met his agreed upon payment arrangements", which he did not Please see faxed copy of signed financial agreement dated 09/05/with pay dates of 10/12,10/26,11/09,11/23,12/and 12/ If you also see patient ledger that was faxed these dates were not met I was having to call and ask for money each time because they did not come in with their payment on the dates stated Our office has a policy that payment is due at the time of service, *** signed this agreement (see faxed documents) We have seen *** anytime he has asked to be seenWe have also allowed the *** to make payments to us and we have carried this debt so to get *** out of pain This is not our office policy, normally we have people seek outside financing, but were told by the *** they would not qualify for this. Star Dental has never refused treatment to *** *** tooth should not be giving him discomfort because we did perform the root canal that he needed to remove the nerve of the tooth and thus relieve any discomfort *** was seen in our office on 03/25/2016, a new impression had to be taken because the other crown that was made did not fit properly This was done at no cost to the patient even though Star Dental will incur expenses for this *** is scheduled back to complete his treatment on this tooth on 04/15/

We only went down to one payment a month when they insisted on doing automatic withdrawal from our account. Additionally, if they saw me anytime, why did I have to contact Revdex.com in order to get service? Once lack of coverage was discovered we agreed to a payment plan. Regardless of Star Dental not taking responsibility for their inaction to this point. They did see me on March 25, 2016 to attempt to complete service. The appliance did not fit and does have to be sent back. This is not my fault and should not be charged (as they indicated). I do have a follow-up appointment in order to complete the work. Which is the main point of the complaint, and if completed as scheduled will resolve the complaint. Although they are taking action finally to resolve the matter. They have a history of canceling appointments on Friday afternoons. I will not consider this matter resolved until the work is complete.
