(non-automatic payment) for either not having a credit card on file or not having the authorization form signed. According to the notes in our system, we can see several follow ups (calls, voicemails and emails) during the month of November thru March. The email we have on file is different from the one provided to the Revdex.com. In December of 2015, due to a traffic violation in 2014 we needed it to rewrite his car under different rate level and DRL.  The rewrite will have an additional charge for the undisclosed accident during the time of quoting. In order to do that we need the policyholder’s authorization.  Several calls were made to explain the situation and to ask for permission for the rewrite. We received no answer during working hours nor after hours (24/7 customer service that can answer any questions or help policyholders with anything they need). Last payment received was in November and the policy went into cancellation effective December 26, 2015. Renters was not cancelled as they are both different line of business hold to different standards. By this point we called, emailed and sent mail trying to communicate with Mr. Paz about the situation. On March 7th he sent an email asking for his renters to be cancelled. Please let me know if I need to submit our notes/ records/ and emails for this matter’s dispute. Regards, Maria L[redacted]
