1/2/Patient called to schedule an eye exam, informed us he has Vision Service Plan insurance, which we are a participating provider.1/7/Patient arrived, checked in at front desk, staff verified insurance coverageNew patient Eye Health Exam performed by *** ***, O.DThis
charge is $115,00, a covered service under his VSP insurance,1, Patient informed doctor he was interested in contact lenses, Additional steps were performed by Dr., *** to lit patient for contact lenses.2, pair A O multifocal trial contacts given to patient, he was instructed on the wear and care of these lensesHe was successful with his trainingHe was asked to wear the lenses and return for a contact lens check in about weeks,1/23/Dr, *** performed a Contact lens check on the “fit* of contacts he received on January 7, Patient was not happy with the clarity of the visionChanges were made and patient received different {lefts & right) multifocal contact lensesDr*** also ordered an additional *** pair of multifocal lenses which we did not have in stockPatient was asked to try these lenses, monitor their comfort and his vision and return for another contact lens check/25/Patient picked up *** contact lenses ordered from 1/21/for himChart was put in our hold bin while patient tried the newest pair of contact lensesPatient did not return to have these contact lenses checked,2/27/Patient called to set up an appointment with the Opticians to get a pair of reading eyeglasses, he told them contacts were not working out,2/28/Patient ordered reading glasses, he was giving up on contact lensesPatient wants to use his VSP insurance towards the purchase of reading glassesHis Rx glasses totaled $200.00, with his VSP coverage his out of pocket expensewill be $plus his co-pay $= $42.00, 3/4/Patient called eyeglasses are ready for pick up.3/10/Reading glasses dispensedOverage on glasses collected ($42.00) at time of dispensePatient made complaint known to front desk staff at this timeHis fitting fee was paid out of pocket and his glasses submitted to his VSP insuranceThe staff knows that if patient gives up or cancels contact lenses their payment($95.00) is non-refundable.The $charge is our charge for all patients who want to be fitted for contact lenses, it includes the follvisitsThis charge was added the day of the examAgain the staff is not sure if he ever questioned this charge or the detailsThis fee would have been discounted by his VSP insurance, since it is an additional expense and is considered under his CL Exam ServicesProviding he continued with the contact lenses and then all charges would have been submitted to his VSP insurance, The patient's plan would have given him $150,for the CL Materials.When I asked Dr*** and the staff about the $charge they could not guarantee he was told of the extra charges for a contact lens fitting he requestedThese services were added to his bill when the patient requested to be fitted for contact lensesIt is shown as an addition to his eye health examSee statement enclosed the fee was paid by the patient.As you will see we were following his Vision Service Plan insurance plan since at the initial start of this process, thinking he would be getting contacts lensesHis VSP plan covers the exam, but any contact lens service or materials are then discounted and patient is responsible for the overage.On January *** *** then ordered eyeglasses and his VSP insurance was now used for this service instead.Needless to say we are surprised to receive a letter from Revdex.com, We are sad to read *** was not satisfied with our service, exactly the additional chargesHis insurance does not cover everything but the staff really tried to do their best to help him get the best out of his insurance plan for the services he received.Should you need any additional information on this patient’s complaint, please contact us at the above telephone number, Thank you

I am rejecting this response because:
I do not dispute the information contained in the response from the Doctor's office.
My point was, and continues to be, that I was not informed of the $95 charge prior to receiving that service.  
Had I been informed of the pending charge, whether to my insurance carrier or myself, I would have had the opportunity to decide if I wanted the service or not.
I think this was an unfair business practice and therefore I should be reimbursed for the charge.
