To Whom It May Concern:       As previously stated  in the written statement the patient [redacted] was never denied medical records. [redacted] claimed identity theft  and he was requested to present himself to Dr. Klein's office with his identification card and sign medical release forms. In addition to pay the fee of $173.25 in cash only no other form of payment will be accepted. Attached are documents; the letter previously submitted to the Revdex.com, [redacted] Identification card, The complaint filed by [redacted] to [redacted] dental Insurance, two requests from Chase in regards to his stop payment on his Chase Debit Card and Emails from Firms Collection agency where [redacted] Claimed Identity theft. have reviewed the response made by the business in reference to complaint ID# [redacted], and have determined that my complaint has NOT been resolved because:I'm still being denied access to my dental records. Now it's false allegations claiming history of disputing my dental claims, [redacted] Insurance company shows no such claim history.  Claims to [redacted] on my behalf, against Stephen Klein, wasn't submitted until February 2017, along with a claim to the Revdex.com, which you received. The bone grafting procedure included in the insurance claim submitted to [redacted] wasn't done by Stephen Klein. The investigation by [redacted] shall show the evidence proving my case. I have received the response from the office of Dr. Stephen J. Klein, which i'm rejecting at this time.  I'm currently following up with my insurance company [redacted]. There is an active grievance with [redacted] out against Stephen J. Klein and i'm waiting for all medical records to be submitted to [redacted] from the dental office. The case is still open and pending documentation from the office of Stephen J. Klein showing services rendered including X-rays. The American Dental Association has also been contacted about this matter.  The office of Professional Discipline is currently looking into this matter as well. Complaint forms have been submitted. It's a violation to deny a patient access to his dental records. Now as far as the money allegations, Stephen J. Klein has been paid by [redacted] period. Payment has nothing to do with allowing a patient access to his dental records. New York State law requries dentists to to allow patients access to their records.
