I am rejecting this response because: It was never explained to me that the one year period had ended until after the Feb visit when I received a bill this is the first time that I have ever received any notice about when this one year period would end Had I known that the Feb visit would be charged, I would have canceled the visit and received my care from my regular dentist

This patient is correct that Periodontal maintenance visits were covered under the fee for his LANAP treatment for a period of one year.  This treatment commenced on January 7, 2015.  There the charge of periodontal maintenance on February 24, 2016 was correct because the one year period...
