I am rejecting this response because:
So first & foremost attached is a timeline of paper work
that I have documented from
the very first phone call I ever made to Stone
Falls to the 2nd to last bill they have sent me***'s statements and
recollection of events are completely falsified and just plain fabricatedThis
is why I waited for their response before producing these hard copies of
evidence
So First I will address the documents & again tell my
side of the story, then debunk the nonsense *** has 98% made up that's just
NOT true
On 03/02/
I called Stone Falls
to ask about the $welcome examx-rays, & cleaningI spoke with Stacy
She said if I just got the exam & x-rays the visit that I scheduled &
attended on 03/24/was
FREENext you will see a welcome letter sent to me by Stone
Falls***'s signature is no
where on this letter sent to me(once again is proof her accounts are fabricated
or she just wasn't a part of this)On my appointment on 03/24/nothing was ever mentioned about my
gums, that in anyway shape or form they where compromised or that I had
periodontal diseaseThe treat plan as you can see is regarding a chipped molar
that needs a crown & possible root canalI did not have insurance at that time
& Stacey as you can see tried to set me up with docpay(a payment plan they
offer)I decided after doing research it was better to purchase insurance
& wait the year to address the chipped molar as it was NOT symptomatic at
the time nor is it til this very day
Fast forward one year My insurance kicked in on 05/01/My wisdom tooth was not symptomatic
but I had chipped my #wisdom tooth & felt that the #wisdom took precedence
over my chipped molar(again which is still not symptomatic)
I called Stone Falls
to make an appointment to Just remove the #wisdom tooth on 05/04/& asked with my
insurance what would my total out of pocket be & again I did not speak with
***I spoke with a much younger, nicer, cuter girl that quoted me $
I replied as I said in my original complaint are you sure that is all my out of
pocket will be? She(younger girl) said, "call the day before, I will ask
the dentist if she can pull the wisdom tooth"I called the day before
& spoke with the same young girl & she replied, "yes the dentist
said, "she can do it"I again asked, "So my complete out of
pocket will be $156.50"? and again she replied, "Yes"
Next attached is my payment to Stone
Falls as stated in my complaint
& now my responseI even took notes on the Full Mouth Debris, Scaling
& Root Planning, & that I would not get my regular cleaning.(this is
where I knew things were going south)They started talking to me about when I
was leaving I needed four cleanings a year not covered by insurance & that
Oh by the way the dentist did a root tip extraction not a simple tooth
extraction & that I "MIGHT" have to pay a few extra bucksSo a
few extra bucks is not $As I mentioned in my complaint, I am disabled & almost blind on limited
income & that an extra $would be a hardship on me)And as I explained in
the complaint I felt very uncomfortable & knew I needed a 2nd opinion on
this matter
Next you can see I left with my referral regarding my
reaming wisdom teeth that day the same day of my appointment on 05/04/2017(
never on 3/24/as *** stated in the response, because I never discussed
my wisdom teeth on 03/24/& never spoke with or saw *** back then, I
only dealt with Stacy)The only thing *** is right about is that I did not
show up to my follow up visit, because I knew that they were scamming me & I
was not going to give them anymore opportunities to try to scare me with their
Tom Foolery & Fear Mongering & do a quick Cash grab to take advantage
of meAgain I never wanted to go back to such a place that did such a horrible
job of salesmanship, giving me information with scare tactics
05/10/
I received the claims they submitted to *** *** my insurance company &
called to ask them(*** ***), "Do I owe anymore money?" &
they replied, "Not to us, but Stone Falls could bill more"I had
explained to them how much I paid & that, that's what they had quoted meI
have appealed the claim with *** *** (since I & my girlfriend who
witnessed this all & is willing to give a SWORN statement), because I/we
feel they have fraudulently filed a claim for which I did NOT consent to a root
tip extraction
Next attached is the treatment plan that I got on 06/15/from a different dentist
at *** *** DentalAs you can see to back up my complaint there is no
mention of periodontal disease or a plan to do any gum work(Full Mouth Debris,
Scaling & Root Planning, & cleanings a year NOT paid by insurance)I
will fill out any & all forms for my complete exam, regular cleaning(that Spring
Valley gave me), & treatment plans(from *** ***) to fully disclose my truth in this whole matter
Next is the 2nd to last bill that Stone
Falls keeps sending me that is in
supposed collections that I called
*** aboutI asked where is the information to dispute this with themShe
said she would get it for me, but never didShe claims I kept talking over
her, but she kept continuing to laugh & say I told you I would send it to
collectionsShe hung up on me(very professional)that's when I decided to
file a complaint with the Revdex.com, appeal the claim & am going to seek all
legal avenues to expose the fraud, abuse, & corruption she has been
imposing on meI will be writing the Illinois Department of Insurance in Chicago
& Springfield
So now let's address each paragraph of ***'s response,
since there is now hard copy proof of my interactions with Stone Falls &

never went there then for wisdom teeth on 03/24/I went there for a chipped molar
Don't know if she called, but I wouldn't have answered
anyway
I did call, but the conversation was a lot differentI
said I feel uncomfortable, because you guys are refusing to give me my
semi-annual cleaning that s included in my insurance plan & I will not be
returning to Stone Falls & no I will not give you a deposit, because I will
not be making anymore appointments with you
I never said send it to collectionsI simply stated I
never agreed, consented or was made aware that there was ever a different
procedure done until after the fact & I was in the check out areaThey
didn't put me to sleep, just local anestheticI think I would of remembered
that
I did call to ask them what the information of the
collection agency wasAs you can see on their Past Due NoticeThere is no
info for a collection agencyI did let them know at that time I would be
filing a complaint, appealing their claim, & notifying the IDI(Illinois
Department of Insurance)That's when *** continued to laugh hysterically,
mocking me, & then hung up on me
The treatment plan that was created on 03/24/as you can see had nothing
to do with wisdom teethOn 05/04/
I was referred out for wisdom teeth 16, 17, & I did call back because
the oral surgeon wanted a $consultation fee just to get in the doorI
said, "that is way too much"This was all within an hour of leaving Stone
Falls on 05/04/& I was still in shockpanic &
frazzled how they had treated me & in all honesty I should have never
called them back to see if the dentist would extract my wisdom teeth at Stone
Falls*** has that note listed
as on 10/05/LOL! We
moved to Missouri on
10/01/2017...hahaaaa Nothing but a lieThis all happened in late April of
& my appointment was on 05/04/
This month of October after our move on 10/01/I have switched to *** *** of Missouri
Her lies & fabrication of made up events are beyond laughable, it's
disgusting
I was never made aware of the change in procedure until
the check out
In conclusion this can all be squared away if they just take
accountability & change my balance to $owedLet's call it a dayOr I
can continue to file complaints all the way to the Illinois Attorney General
& file a complaint with the ADA,
ACLU, & have our family attorney file a claim of conspiracy to commit
insurance fraud
