I am rejecting this response because:
As stated before, I was verbally told that I will have no co-payI was told this at least timesHow can you charge me for something I never received? You are guilty of insurance fraud as well as consumer fraudif no resolution is made, my next step will be to contact the Ohio Attorney General and the American Dental AssociationI have researched this type of practice in detail and you are in violation of several State ordinancesthe contract I signed was under duressI find it odd that you have not addressed what the Dentist told me

Initial Business Response /* (1000, 5, 2015/06/17) */
Sara,
I am sorry that your visit did not meet your expectationsAfter looking at your treatment and case closely, I have seen that you were billed for a root canal (which was completed successfully) and a buildup on the same tooth (which was
also done successfully) and for a crown that was prepared and a mold was taken and sent to our lab to completeYour next visit is scheduled to have the crown seatedIt will be finished successfully as well should you choose to returnDoing the crown in two visits is and your next visit will complete your treatment on this toothYou have paid for this treatment and it will be completed wonderfullyNo treatment was performed on any other tooth in your mouth and nothing was billed eitherYou received what you have paid forThere is no refund necessary
As for your complaint about rushing and pain, I can only tell you that you received the proper amount of anesthetic for the treatment performed at each visit and when you did ask for more anesthetic it was given to you in a different manner to help treat your infected toothSome procedures are more painful than others and we strive to provide all of our patients with a pain free experience, but nonetheless, presenting to us with an infected tooth can have its challenges, no matter who the provider is treating youPrior to these two visits we had not seen you in our office for yearsWe wish we had seen you sooner and been able to try and prevent a root canal from being performed
At this point you should return to our office and have your crown finished and then make a decision whether you choose to see us in the future for the rest of your treatment for cavities that you do have in your mouth
Your request for a refund is not warranted as all procedures have been performed properly and we will finish things and deliver your permanent crown on your next visitShould you choose to not move forward and receive your permanent crown we can not guarantee the root canal as your are only in a temporary crown and that has an extremely limited lifespan (several weeks only)If you would like to request another provider finish your case in our office we can have that arranged, but EVERY visit you have been seen for in our office has been with the same doctor, even years ago and this is the first complaint you have ever expressed to our office about your treatment
Please feel free to contact us if you are in need of any further assistance and we hope to see you soon to complete your treatment

Thank you for your response Mr. [redacted].  As we stated in our initial response, we will honor the payment in full process for the next short Tim period and then we will proceed with our usual and customary collection process when a balance is not paid in full for services rendered.  Thank you for acknowledging that you did sign a financially binding contract and for having the services that you approved delivered to you, in full.  We wish you the best.   Dr. Jeff R[redacted] - Streetsboro Dental

Initial Business Response /* (1000, 5, 2016/05/26) */
I have several concerns with this inquiry.
1. I am not sure that this is the actual patient contacting us or it is the patients grandson who I spoke to yesterday and informed that no refund would be given and he abruptly hung up on me due to...

his disappointment.
We are not permitted by HIPPA laws to discuss treatment with another family member unless authorized to do so by the actual patient, in writing. This actual patient that is being referenced is located in California at this time and has been there for some long period of time as well.
2. The treatment was performed over 1 year ago and during it the patient approved all steps and was satisfied with the results and was also recommended to have some follow up treatment performed to continue to improve the device that she was given. The patient did not follow thru with treatment and then over a year later, we hear from the grandson that he requests a refund from this patient.
3. In the conversation with the grandson I asked him to have the patient brought to our office to evaluate the concerns and then we would be delighted to evaluate any situation that he says occurred. He informed me that this was not possible and thus I told him that we could not offer a refund for a procedure that was performed correctly, done with exceptional care and quality and then a year later he has a concern.
I consider this matter to be closed as we were told that the patient would not be returning to our office to evaluate the situation. We are not going to violate any HIPPA laws by speaking to another family member without authorized consent to do so from the actual patient. We need this in writing to do so. Furthermore, the patient did not follow thru with advised treatment as recommended and according to our records it would have a direct impact on the success of this device given to the patient for its long term success.

We are fully aware of Mr. [redacted]'s complaint.  We have also notified him of our response when he called our office and asked the same questions and was given the same answers.   Although it is not attached to this email, we have a signed document indicating that Mr. [redacted] signed...

informing him of his co-payment for the services that were rendered.  It can be produced at your convenience and I am sure Mr. [redacted] has it in his capacity as our patients receive a copy of this when they sign it and enroll and approve of the treatment. On this note as well, Mr. [redacted] informed us that he "did not read it" and thus he thought he was to have no co-payment for the services.Mr. [redacted] has a dental plan and he chose to come to our office on his own accord.  It is not our dental insurance plan and it is not our responsibility to know the details of this plan.  It is a courtesy that we provide to our patients to assist them in their concerns.  When Mr. [redacted] chose not to pay attention to his plan and then chose not to pay attention to his signature on a financial document and then had an outstanding bill he chose to call our office and state that he should not have to pay. We stand by our initial decision that the balance will be paid in full within the next 10 days or Mr. [redacted] will be sent to our collection agency so that we proceed with the payment in full. Sincerely, Dr. Jeff R[redacted]
