*** *** completed OSHA-Training with Sudden Safety Consultants at our old address, *** *** *** *** ** ***, on August *** , That was a SundayWe provided our standard Completion Letter that confirmed *** ***’s attendance, completion date, and topics covered so he could successfully seek employment the following dayThe bottom of this same letter states:
“This class will be documented to my authorizing OSHA training organizationUpon successful approval of my documentation, receipt of required photo, and any balances due to the trainer, each student will receive a laminated OSHA completion card within days of the end of the class. Under NYC Local Law 41, the ten (10) hour OSHA card has a five (5) year expiration date.”
The completion letter is read to each student individually as part of the “exit strategy” before they leave the premises. OSHA’s terms for delivery of OSHA cards is also explained in detail as part of the Opening presentation before safety class gets started
*** *** called to inform my assistant that he had been turned away from his jobsite because they would no longer accept his letterMy assistant explained OSHA requirements, but *** *** wouldn’t accept thatWhen asked who he reported to so we could confront them, he wouldn’t sayWhen asked the name of the person that turned him away, he couldn’t sayWhen asked to come in to give more information, he refused, but kept calling incessantly demanding an immediate OSHA cardWe cannot produce immediate cardsNo legitimate trainer can! We also cannot run a business with one person franticly tying up our phones but not cooperating with our requests to come in
Per the attached OSHA Outreach Training Requirements, we have days to get cards to the studentsWe are a person operationUnlike the larger training organizations, we are able to keep our prices low because we aren’t staffed up as they areWe still report to OSHA in a timely fashion, however, and respond rapidly to those incidents where an inspector may not be familiar with usWe then complete the cards as quickly as possible
Days later *** *** called me after hours directly on my cell phoneWe kept getting cut off because there is poor reception when driving on the ParkwaysI live in Connecticut, which is a hour driveIt’s difficult to maintain ANY conversation during that rideOnce home, I reiterated to *** *** that he needs to come in to my office directlyAlthough I tried to empathize with him and assist, he became more combative and insisted on calling all hours of the nightThat’s harassment! *** *** began to disrespect my personal boundaries
Eventually I did get him to come in during our regular office hours on Tuesday, September ***. The letter he presented me was a complete messIt was faded, frayed, tinted with clothing dye, and appeared to be, as the inspector told him, “a copy that looked like fraud”I provided *** *** with a NEW letter, which was stamped with my company seal and signed in original inkI advised him to make a copy and keep the original in good repair. I also showed him a copy of the pertinent section of OSHA Outreach Training Requirements, which I recited to his supervisor who we reached by phone while he was hereThe attached email to the NYC DOB ensuedIt cites the OSHA requirements to NYC DOB and explains the reasonable expectation that the *** ***, Supers, *** *** and *** *** *** will accept valid temporary letters and certificates on ALL worksites at all times
All the while I was fighting on *** ***’s behalf to get him restored to the site, I could never have guessed that he would deceptively file a complaint with such horrific accusationsI am years old and have worked tirelessly in construction in this neighborhood where I grew up since While I was willing to stick my neck out to make a few enemies at NYC DOB on behalf of my students, it seems many are impatient and self serving to the point where they are willing to make accusations to their advantage*** *** is part of a re-entry program that paid for his feeNone o the other participants nor program leaders have complained
Perhaps *** *** didn’t consult others about my reputationI am an Authorized OSHA trainer listed on the OSHA website by the Federal Department of LaborI am directly referred to most students by the OSHA Office on *** *** in ManhattanThey can attest to the OSHA requirements too
Today I received a call from another jobsite, as others have done, verifying the OSHA completion status of another person who trained with me recentlyThere was no problem there. Maybe the problem was how he maintained his letter, or maybe something else. I can only imagineI wish him luck in the real world

We received an email stating that on March **, she would be giving us the card numbers for our employees who attended the OSHA training sessions. As of this date (3/**/15) we have not received the card numbers or the cards. We have not heard any further from the trainer

Review: I received training from this business in OSHA-10 in April of 2015. The service was paid in full and in advance, all the documents provided. Basically, all requirements were met.

They advertised the issuance of the card within 90 days from date of service. BTW the card is valid for 5 years.

It has been over 8 months since I took a course, but I have never received my card. They don't send any updates, and to me it seems that they just forgot about my card, or lost it.

My right to use the card for 5 years was neglected.Desired Settlement: I would like to get my card ASAP. I would also appreciate an apology.

Review: ESH hire company to facilitate an OSHA training on March ** and **. Vendor was paid in full $1200 upfront. Classes were conducted for 10 participants, however, no one has received their OSHA cards. ESH has made at least 20 (inclusive of phone calls, emails, and site visits) attempts to pick up the cards since April 2015. Our last attempt was on October * and * via phone and message was left with the office clerk.Desired Settlement: ESH would like a refund for we have exhausted all options in getting cards for our members. This transaction has damaged relations with some members and we are working to regain their trust and participation.

Review: We enrolled our employee [redacted] to take the 10-Hr. OSHA class on 10/**/2014 & 10/**/2014. We paid the fee of $160.00. Our employee needs the card to be able to access the jobsites. The 10-HR OSHA card is issued around 8-10 business days after the course is completed. However, he never received his card. Numerous attempts have been made during 7 months requesting his OSHA card but up to this date this hasn't been resolved. At first, our emails and calls were ignored, then on 3/**/2015 we received a reply from [redacted], the [redacted] of the company. In this e-mail he stated that instead of the card they would issue a letter of completion. The problem is this letter was not accepted by all our contractors and in different occasions [redacted] wasn't able to work. Since he still didn't receive his card, we had no choice but to enroll him to take the class again with a different company. We requested a refund since we didn't receive what we paid for but [redacted] from [redacted] refuses to issue a refund.

It is unacceptable that we paid for a service and we still haven't received what we paid for not 2 or 3 months later but 8 months later.

We are asking you to please reach out to them, you shouldn't provide a service if you are not in the position of fulfilling it. This is not the way to do business.Desired Settlement: Full refund in the amount of $160.00 USD

Review: I contacted Sudden Safety Consultants after finding them listed on [redacted] which I was directed to from the [redacted] website. I took my 10 hour OSHA class in September 2014. I received a letter stating that I completed the course but I never received my OSHA card which is what I paid for. I have contact them at ###-###-#### several times and spoke with a lady who informed me that the business was audited and my card should be in the mail shortly. After doing some searches I see that other people have experienced the same issue with the business and I am very displeased. I am hopeful that we will all get our money back and this business and number associated with the business will be shut down. This is less about the money and more about misleading people who need this certification to provide for their families. This has cost my family more than the value of the certification and I am very unhappy. I am escalating this matter to the Department of Labor as [redacted] is also not helpful or cooperative and should not be listed as a source for providing people with accurate centers to receive OSHA cards.

I hope this helps someone else who is looking for a training center. Do not do business with this organization.Desired Settlement: I would like my money refunded. I would like this company's website to be shut down since they are not providing people with actual certifications.

Review: Sudden Safety Consultants, via [redacted], provided a ten hour OSHA construction training course to some of our employees. The training took place in our office on 10/* & 10/**/2014. It is required that each attendee receive an OSHA card within 90 days of taking the course, however it's possible that the cards can be processed in less time. We began inquiring about the cards in December of 2014. [redacted] said she had been out of the office for a couple of weeks due to the holidays. Further follow up calls finally resulted in [redacted] admitting that she was being audited, due to errors in reporting, and that she did not have the cards yet. She responded to a letter from us on February [redacted] (via email) stating the cards would be sent to us within 2 weeks. As of this date (2/**/2015) we have not received the cards.Desired Settlement: Simply put, we would like to receive the OSHA training cards to close out this matter.

Review: Gm I actually put my complaint in the next box below. Trouble copying n paste my Apologies. I would like a refund or my 10 hr Osha ASAP.

Thank youDesired Settlement: I took an 10hr Osha course which I paid for and completed back in May 2015. I extended myself and been very patient with Ms. G[redacted]. I have made extremely to many attempts by stopping by which I'm in walking distance as well by p hone and tex. I also lost several employments opportunities because I didn't have my Osha card. As a resolution I would like my card expeditiously or my refund so I can re take the course somewhere else. She also charged an extra $10 stating Osha ne policy is pictures are now a must. Do not come to her office because she will not release the cards unless you paid for picture. I actually looked into that it is not correct .information that she is providing to her clients. A way for her to hustle more money out of customers. Shame on her treating people like that in community. Who are not in a place to come out of pocket or desperately seeking employment like myself.

Review: [redacted] is suppose to be a legit trainer teaching the 10 and 30 hour construction classes to obtain the OSHA card, who has only proven to be a fraud. She gave me a print out saying that I completed my 10 hour OSHA after I completed a 2 day training, and then had me pay for a photo that she said would go on my Osha card. I was able to get a construction job, upon inspection I was told that the paper given wasn't acceptable and that I was suppose to receive a certificate. I had then tried contacting [redacted] only for her to pick up the phone and hang up on me several times after I told her what I was calling about. Then I found out from others that she did the same thing to them, taking their money and giving them a print out, nobody received a Osha card, and the Osha card does not have a photo on it. [redacted] is a very old woman stealing people's money who wants to do something with their lives. However a the if has no age limit.Desired Settlement: I want what I paid for, what I went and sat in for to train for, my 10 hour Osha. Now I can not go back to work until I get it, if they still have that position available to me by the time I do get it.
